刘喜明运用攻痰逐饮法治疗癌性腹水经验
Professor Liu Ximing's experience in the treatment of cancerous ascites with the method of clearing up phlegm and expelling fluids

Abstract
Cancerous ascites is a significant complication in the advanced stage of malignant tumors.Its formation is primarily attributed to the increased permeability of peritoneal blood vessels due to the secretion of certain mediators of tumors,or factors such as imbalance in peritoneal fluid dynamics,portal vein obstruction,liver metastasis,and obstruction of lymphatic and venous returns.It falls within the scope of traditional Chinese medicine(TCM)patterns such as"tympanites","suspending fluid retention"and"phlegm and fluid retention".It is characterized by the pathological features of trapped and difficult-to-resolve phlegm-toxin,hindrance in the circulation of qi,blood,and body fluids,and accumulation of pathogenic dampness.In the case of deeply trapped and congealed phlegm-toxin,only can powerful and high-dose medicines effectively eliminate it,while for the stagnation and amassment of pathogenic dampness,only can dredging and purgative methods be adopted to thoroughly dispel it.Professor Liu Ximing has consistently achieved remarkable efficacy in his clinical treatment of cancerous ascites by applying the phlegm-clearing and fluid-expelling method.He emphasizes the use of bitter and pungent herbs,as well as those with a descending nature to achieve the therapeutic effects of clearing up phlegm,expelling fluids,and descending and purging pathogens.The formulation"Kongxian Dan"is recommended as the principal prescription for treating malignant ascites using the phlegm-attacking and dampness-eliminating method.Professor Liu Ximing also points out that herbs such as gansui root,Peking euphorbic root,and lilac daphne flower bud are all potent in their actions and should not be prepared in decoctions to pursue a swift response,which may damage the stomach qi as the medicines act on the affected area,thereby failing to cleanse the deep-seated and hidden phlegm-fluid.Professor Liu Ximing advocates a gradual approach with pill formulations,supplemented by ingredients like ginger juice,American ginseng,and other herbs that nourish and support the spleen and stomach,bolstering the vital qi.Alternatively,herbal decoctions can be administered based on an individualized syndrome differentiation,thereby integrating syndrome differentiation with disease differentiation to treat cancerous ascites,in order for the disease to be cured over time.关键词
