半髋和全髋关节置换术治疗老年移位性股骨颈骨折的10年随访疗效比较

Abstract
Objective To compare the 10-year follow-up efficacy of hemiarthroplasty(HA)and total hip arthroplasty(THA)on the displaced femoral neck fracture in the elderly.Methods Forty-eight elderly patients with displaced femoral neck fractures were divided into HA group(30 cases were treated with HA)and THA group(18 cases were treated with THA),according to different surgical procedures.The intraoperative blood loss,operative time,and modi-fied Harris hip scores(MHHS)were compared between the two groups.Results All patients were followed up for 10 years.The intraoperative blood loss and operation time in the HA group were less(shorter)than those in the THA group(P＜0.05).During the ten-year follow-up period,no postoperative situation such as prostheses dislocation or re-vision occurred in both groups.At 10 years after surgery,there were no statistical differences in pain,function,activity and total scores of MHHS between the two groups(P＞0.05).The patients of both groups could walk slowly with crut-ches.Conclusions For treatment of elderly displaced femoral neck fracture with HA and THA,although HA has ad-vantages in intraoperative bleeding volume and surgical time,there is no difference between the two procedures from the long-term effect of hip function recovery.关键词
