Abstract
Objective To compare the effects of direct anterior approach(DAA)and posterolateral approach(PLA)in total hip replacement(THA).Methods The 101 patients were divided into DAA group(53 cases were treated by DAA approach THA)and PLA group(48 cases were treated by PLA approach THA)according to different approa-ches.The operation time,incision length,intraoperative blood loss,postoperative drainage volume,postoperative hospitalization time,hospitalization cost,pain VAS,Harris score of hip joint function and complications were recor-ded.Results All patients were followed up for 6 months.There was no statistical difference in operation time be-tween the two groups(P＞0.05).The incision length,intraoperative blood loss,postoperative drainage,postopera-tive hospitalization time and hospitalization cost in DAA group were shorter(less)than those in PLA group(P＜0.05).Pain VAS:DAA group was lower than PLA group at 1 and 3 months after operation(P＜0.05),and there was no statistical difference between the two groups at 6 months after operation(P＞0.05).Harris score:DAA group was higher than PLA group at 1,3 and 6 months after operation(P＜0.05).All the prostheses were in good position after operation,and no surgical complications such as incision infection,dislocation of joint prosthesis,hip pain and pressure sore occurred.Conclusions Compared with PLA approach,DAA approach has the advantages of smaller incision,less bleeding,shorter postoperative hospitalization time and lower hospitalization cost,which can effectively relieve patient's pain in the early period of the postoperation,and improve joint function.关键词
