锁定钢板内固定治疗肱骨近端三、四部分骨折的早期疗效
Early efficacy of locking plate internal fixation in the treatment of 3-and 4-part proximal hu-meral fracture
摘要
Abstract
Objective To investigate the early efficacy of locking plate internal fixation in the treatment of 3-and 4-part proximal humeral fracture.Methods The 51 patients with 3-and 4-part proximal humeral fractures were treated by locking plate internal fixation.The postoperative complications,fracture healing time,pain VAS and Constant-Mur-ley score were recorded.Neer score of shoulder joint was used to evaluate the clinical efficacy.Results All patients were followed up for 6 months.During the follow-up,fracture,deformation and displacement of the locking plates were not found;there were 8 cases of postoperative complications,including 3 cases of joint stiffness,1 subacromial im-pingement,2 screw loosening,1 poor fracture healing and 1 delayed healing of incision,and the complication incidence rate was 15.69％.All the fractures healed within 3～6 months.Pain VAS,Neer scores and Constant-Murley scores:they were all better at 6 months than those at 3 months after surgery(P＜0.05).At the last follow-up,clinical efficacy was evaluated using the Neer score:excellent in 19 cases,good in 25,fair 6 and poor in 1,with an excellent-good rate of 86.27％.Conclusions In the treatment of 3-and 4-part proximal humeral fracture,locking plate internal fixation can maximize preservation of bone mass,significantly reduces the impact on blood supply at the fracture site.It is ob-viously helpful to fracture healing and shoulder joint function recovery,with good early clinical efficacy.关键词
