Abstract

Objective:To comprehensively evaluate the efficacy and mechanisms of Dachaihu Decoction(DCHD)in different types of cholestasis animal models.Methods:A systematic search was conducted in PubMed,Cochrane Library,Embase,China National Knowledge Infrastructure(CNKI),China Science Periodical Database(CSPD),and Chinese Sci-tech Journal Database(CCD)from their inception to April 30,2023,to identify published animal studies on DCHD intervention in cholestasis.Two researchers inde-pendently screened reports,extracted relevant data,and assessed the risk of bias using the Systematic Review Center for Laboratory Animal Experimentation(SYRLE)tool and the quality of evidence using the GRADE-CERQual approach.Results:A total of 15 an-imal studies were included.Except for one study reporting that DCHD failed to alleviate bile duct ligation(BDL)-induced cholesta-sis,all other studies demonstrated the effectiveness of DCHD in various cholestasis models.The evidence supporting DCHD's effica-cy in extrahepatic cholestasis models was stronger than in intrahepatic models.In the analysis of DCHD's mechanisms,the quality of evidence for"inflammation"was rated as high,while"bile acid homeostasis"and"apoptosis"were rated as moderate.Mean-while,"oxidative stress""intestinal mucosal barrier",and"endoplasmic reticulum stress"received low-quality ratings.Conclu-sion:DCHD has a hepatoprotective effect and alleviates cholestasis in different animal models of cholestasis,with stronger evidence supporting its effects in extrahepatic models.Mechanistically,its anti-inflammatory effect has the highest quality rating,whereas its role in regulating bile acid homeostasis,apoptosis,oxidative stress,intestinal mucosal injury,and endoplasmic reticulum stress has relatively lower evidence quality.
