腹腔镜与开腹手术治疗小儿穿孔性阑尾炎的临床效果比较

Abstract
Objective To compare the clinical effect of laparoscopic and open laparotomy in the treatment of perforated appendicitis in children.Methods From January 2017 to December 2022,169 children with perforated appendicitis in the Department of Pediatric Surgery,Children's Hospital of Changchun were selected and divided into two groups according to different surgical methods.Among them,79 cases were treated with appendectomy via open laparotomy(open group),90 cases were treated with laparoscopic appendectomy(laparoscopic group).The clinical effects of the two groups were compared.Results The operative time of laparoscopic group was longer than that of open group,and the proportion of indwelling drainage tube was lower than that of open group,the differences were statistically significant(P<0.05).There were no significant differences between the two groups in the first postoperative exhaust time,the first postoperative eating time and the length of hospital stay(P>0.05).After surgery,the incidence of incision infection,incision liquefaction and intestinal obstruction in laparoscopic group was lower than that in open group,the difference was statistically significant(P<0.05),and there was no significant difference in the incidence of pelvic abscess between the two groups(P>0.05).Conclusion Compared with appendectomy via open laparotomy,laparoscopic appendectomy in the treatment of perforated appendicitis in children can reduce the insertion rate of drainage tube,and the incidence of postoperative intestinal obstruction is lower,which is worthy of promotion.关键词
