电针联合穴位贴敷治疗带状疱疹后遗神经痛的疗效观察
Clinical Observation on the Efficacy of Electroacupuncture Combined with Acupoint Application in Treating Postherpetic Neuralgia

Abstract
Objective:To observe the clinical efficacy of electroacupuncture at Jiaji(EX-B2)combined with acupoint application in treating elderly patients with postherpetic neuralgia(PHN)of qi stagnation and blood stasis type,and to evaluate its effects on T cell subsets(CD3+,CD4+,CD8+)and pain-related factors,including substance P(SP)and β-endorphin(β-EP).Methods:A to-tal of 100 PHN patients were randomly divided into an observation group(n=50)and a control group(n=50)according to a ran-dom number table.The observation group received electroacupuncture at Jiaji(EX-B2)corresponding to the affected nerve seg-ments,combined with detumescent analgesic patches and acupoint application of Chinese medicine fine powder at Sanyinjiao(SP 6,bilateral),Shenque(CV 8),and Ashi points(two locations).The control group received oral gabapentin capsules.Changes in Visu-al Analog Scale(V AS)scores,traditional Chinese medicine(TCM)syndrome scores,Pittsburgh Sleep Quality Index(PSQI)scores,T cell subsets(CD3+,CD4+,CD8+),and levels of SP and β-EP were assessed before treatment and after 4 weeks of treat-ment,and clinical efficacy was evaluated.Results:After 4 weeks of treatment,both groups showed significant reductions in VAS scores,TCM syndrome scores,PSQI scores,CD8+levels,and SP levels compared to pre-treatment values(P＜0.05),and the ob-servation group was lower than the control group(P＜0.05).Meanwhile,CD3+,CD4+,and β-EP levels increased significantly in both groups(P＜0.05),and the observation group was higher than the control group(P＜0.05).The total effective rate in the ob-servation group was 95.74％,significantly higher than 77.08％in the control group(P＜0.05).Conclusion:Electroacupuncture at Jiaji(EX-B2)combined with acupoint application is significantly superior to gabapentin capsules in treating elderly PHN pa-tients with qi stagnation and blood stasis type.It provides superior pain relief,improves TCM symptoms and sleep quality,reduces CD8+and SP levels,and increases CD3+,CD4+,and endogenous analgesic β-EP levels.关键词
