黄芪桂枝五物汤加减方对紫杉醇所致周围神经病变的保护作用研究
Protective effect of modified decoction of five drugs including Astragalus Root and Cinnamon Twig on paclitaxel-induced peripheral neuropathy

Abstract
Objective It is to investigate the protective effects and mechanisms of modified decoction of five drugs in-cluding Astragalus Root and Cinnamon Twig on paclitaxel-induced peripheral neuropathy in rats.Methods Seventy male SD rats were randomly divided into control group,model group,mecobalamin group,original decoction group,modified decoc-tion group,original decoction+mecobalamin group,modified decoction+mecobalamin group,with 10 rats in each group.The rats of all groups except for the control group were given paclitaxel by intraperitoneal injection every other day to estab-lish models of PIPN.Meanwhile,since the 1st day of the experiment,the mecobalamin group was given 1 mg/mL mecobal-amin aqueous solution 1 mL/kg by gavage,the original decoction group was given 2 g/mL decoction of five drugs including Astragalus Root and Cinnamon Twig 10 mL/kg by gavage,the modified decoction group was given 2 g/mL modified decoc-tion of five drugs including Astragalus Root and Cinnamon Twig10 mL/kg by gavage,the original decoction+mecobalamin group was given2 g/mL decoction of five drugs including Astragalus Root and Cinnamon Twig10 mL/kg combined with1 mg/mL mecobalamin aqueous solution 1 mL/kg by gavage,the modified decoction+mecobalamin group was given 2 g/mL modified decoction of five drugs including Astragalus Root and Cinnamon Twig 10 mL/kg combined with 1 mg/mL mecobalamin a-queous solution 1 mL/kg by gavage,the control group and model group were given equal volumes of saline by gavage,all once daily,continuously treated for 21 days.The mechanical pain threshold and heat pain threshold of the rats were detec-ted,the changes of their body weight were observed,the nerve fiber damage in skin tissue of hindfoot sole of the rats were detected by immunofluorescence,and the protein expressions of nuclear factor E2-related factor 2(Nrf2),Kelch-like ECH-associated protein 1(Keap1)and heme oxygenase-1(HO-1)in dorsal root ganglion were detected by Western blot.Re-sults Compared with the control group,the mechanical pain thresholds of rats in the model group on the 6th,8th,11th,14th and 21th days of experiment,and the thermal pain thresholds on the 8th,11th,14th and 21th days of experiment were significantly decreased(all P<0.05),the body weight of the rats increased slower,the average fluorescence intensity of PGP9.5-positive expression in skin tissues of hindfoot sole of the rats was significantly decreased(P<0.05),and the rela-tive protein expressions of Nrf2,Keap1 and HO-1 in dorsal root ganglia were significantly increased(all P<0.05).Com-pared with the model group,the mechanical pain thresholds of rats in the medicated groups on the 6th,8th,11th,14th and 21th days of experiment,and the thermal pain thresholds on the 8th,11th,14th and 21th days of experiment were signifi-cantly increased(all P<0.05),the changes of body weight of the rats were not significant,the average fluorescence inten-sities of PGP9.5-positive expression in skin tissues of hindfoot sole and the relative protein expressions of Nrf2 and HO-1 in dorsal root ganglia of the rats were significantly increased(all P<0.05),and the relative protein expressions of Keap1 in dorsal root ganglia were significantly decreased(all P<0.05).The mechanical pain thresholds of rats in the original de-coction group,modified decoction group,original decoction+mecobalamin group,modified decoction+mecobalamin group on the 14th day of experiment and the heat pain thresholds of rats in the modified decoction group,modified decoction+mecobalamin group on the 14th day of experiment were significantly higher than those of rats in the mecobalamin group(all P<0.05)and the heat pain thresholds of rats in the modified decoction group,modified decoction+mecobalamin group on the 14th day of experiment were significantly higher than that in the original decoction group(both P<0.05).The relative protein expressions of Nrf2 and HO-1 in dorsal root ganglion of rats in the modified decoction group,original decoction+mecobalamin group and modified decoction+mecobalamin group were significantly higher,while the protein expressions of Keap1 were significantly lower than those in the mecobalamin group and original decoction group(all P<0.05);the rela-tive protein expressions of Nrf2 and HO-1 in dorsal root ganglion of rats in the modified decoction+mecobalamin group were significantly higher,while the protein expression of Keap1 was significantly lower than those in the modified decoction group and original decoction+mecobalamin group(all P<0.05).Conclusion Modified decoction of five drugs including Astraga-lus Root and Cinnamon Twig can reduce mechanical and heat pain sensitivities induced by paclitaxel chemotherapy,reduce loss of epidermal nerve fibers,activate the body's antioxidant function,thus to plays a role in preventing and treating pe-ripheral neuropathy caused by paclitaxel chemotherapy,and its efficacy is better than original decoction of five drugs inclu-ding Astragalus Root and Cinnamon Twig alone.关键词
