Efficacy of decoction for fortifying spleen and replenishing kidney on cancer-related fatigue in prostate cancer with syndrome of spleen-kidney deficiency after androgen deprivation therapy

Abstract
Objective It is to evaluate the efficacy and safety of decoction for fortifying spleen and replenishing kidney in the treatment of cancer-related fatigue(CRF)in prostate cancer with syndrome of spleen-kidney deficiency after andro-gen deprivation therapy(ADT).Methods A randomized,controlled,double-blind,placebo-controlled study was per-formed,in which 40 CRF patients with prostate cancer after ADT,and differentiated as syndrome of spleen-kidney deficien-cy in the Department of Urology,Xiyuan Hospital of China Academy of Chinese Medical Sciences from August 2022 to Jan-uary 2024 were randomly divided into experimental group and control group.The experimental group was given granules of decoction for fortifying spleen and replenishing kidney by oral administration,while the control group was given placebo granules of decoction for fortifying spleen and replenishing kidney by oral administration,both groups were continuously treated for 12 weeks.The scores of Brief Fatigue Inventory(BFI),Expanded Prostate Cancer Index Composite(EPIC-26),Aging Male Symptoms(AMS)of patients in the two groups before treatment and after treatment for 2,4,6,8,10 and 12 weeks were compared,the serum levels of prostate-specific antigen(PSA),testosterone(T),safety indicators(blood routine,urine and stool routine+occult blood,ECG,liver function and renal function)of patients in the two groups were detected before and after treatment for 12 weeks,the occurrence of adverse reactions in the two groups was recorded.Results Compared with before treatment,the BFI scores after treatment for 6,8,10 and 12 weeks and AMS scores after treatment for 10 and 12 weeks were significantly decreased,while the EPIC-26 scores after treatment for 6,8,10 and 12 weeks were significantly increased in the experiment group(all P<0.05);there was no obvious changes in the BFI scores,EPIC-26 scores and AMS scores at all time points after treatment in the control group(all P>0.05).The BFI score after treatment for 12 weeks in the experimental group was significantly lower than that in the control group(P<0.05),there was no significant difference in the EPIC-26 scores and AMS scores at all time points after treatment between the two groups(all P>0.05).After 12 weeks of treatment,there were decrease tendencies in serum levels of PSA and T in both groups after treatment for 12 weeks,but the differences were not statistically significant when compared with before treatment in each group and between the two groups after treatment(all P>0.05).One patient in each group withdrew from the study due to diarrhea symptoms after taking the drug,and no other adverse reactions were found.Conclusion De-coction for fortifying spleen and replenishing kidney can alleviate fatigue and adverse effects related to androgen deficiency,and improve the quality of life with good safety in the treatment of CRF patients with prostate cancer with syndrome of spleen-kidney deficiency after ADT.关键词
