Abstract
Objective To analyze the clinical and dosimetry related factors for radiation pneumonia after intensity modulated radiotherapy of breast cancer.Methods Clinical data of 60 patients undergoing intensity modulated radiotherapy of breast cancer admitted to our hospital from January 2020 to December 2024 were retrospectively analyzed.Clinical characteristics and dosimetry indicators of the patients were collected,and they were divided into occurrence group(n=20)and non-occurrence group(n=40)according to the occurrence of postoperative radiation pneumonia after intensity modulated radiotherapy of breast cancer.The correlation between risk factors and radiation pneumonia was analyzed by Logistic regression.Results No significant difference was found in the age,menopausal status,stage,hemoglobin,endocrine therapy before radiotherapy,radiotherapy mode and chemotherapy cycle between the two groups(P>0.05);significant difference was found in the chemotherapy between the two groups(P<0.05).No significant difference was found in the dosimetry parameters(PTV,MLD,V40 and V50)between the two groups(P>0.05);significant difference was found in the V5,V10,V20 and V30 between the two groups(P<0.05).Logistic regression analysis results showed that V5 and V20 were independent risk factors for the occurrence of radiation pneumonia after intensity modulated radiotherapy of breast cancer(P<0.05).Conclusions After intensity modulated radiotherapy of breast cancer,radiation pneumonia is still one of the major complications affecting the prognosis of breast cancer after intensity modulated radiotherapy.V5 and V20 are independent risk factors leading to radiation pneumonia after intensity modulated radiotherapy of breast cancer,so the occurrence of radiation pneumonia can be effectively prevented by focusing on the dose of V5 and V20 in the context of the limitation of radiotherapy dose parameters.关键词
