围术期镇痛合作药物治疗管理模式的建立与应用效果评价
Establishment and effectiveness evaluation of cooperative drug therapy management model based on perioperative pain

Abstract
AIM To establish a cooperative drug therapy management model based on perioperative analgesia,and to evaluate its clinical effectiveness in optimizing perioperative pain management among hospitalized surgical patients.METHODS A total of 1 683 surgical ward cases involving patients who underwent surgery in May,July,September,and November 2023 and received patient-controlled intravenous analgesia(PCIA)postoperatively were included.A collaborative perioperative analgesic medication management model was established and implemented in June 2023,under which clinical pharmacists joined the acute pain service(APS)team to participate in formulating a standardized postoperative multimodal analgesic protocol,provide training to surgeons,and collaborate with anesthesiologists and nursing staff to evaluate analgesia efficacy through PCIA follow-up,monitor adverse drug reactions,and assess the appropriateness of supplementary analgesic prescriptions in the surgical ward.Based on the evaluation outcomes,the Pharmacy Department formulated refined control measures for perioperative analgesic medications.RESULTS The per capita cost of analgesic drugs in the surgical ward showed a continuous downward trend,dropping to 820.50 yuan,605.77 yuan,367.10 yuan,and 316.61 yuan in May,July,September,and November 2023 respectively for 1 683 patients.The rationality rate of medical orders for analgesic drugs increased continuously,reaching 32.27％,45.15％,73.90％,and 93.43％respectively in these months.The incidences of nausea and vomiting were 8.31％,6.42％,3.54％,and 2.71％respectively in these months.There were no significant changes in resting Visual Analogue Scale(VAS)and exercise VAS scores among patients in different months at different time points(P＞0.05).CONCLUSION The combined analgesic mode of opioid and non-steroidal analgesics is helpful to reduce the incidence of nausea and vomiting and reduce the cost of analgesic drugs.The multi-disciplinary cooperative pharmaceutical management mode could promote rational drug use more effectively without compromising analgesic efficacy.关键词
