支气管镜介入治疗气管食管瘘的临床效果分析
Analysis on Clinical Effect of Bronchoscopy Intervention in the Treatment of Tracheoesophageal Fistula

Abstract
Objective To explore the application of stent implantation in esophagotracheal fistula,and to improve the treatment and management of esophagotracheal fistula.Methods A total of 55 patients with esophagotracheal fistula who received interventional treatment with menstrual bronchoscopy in Fuwai Central China Cardiovascular Hospital were collected.The clinical data were analyzed in detail and followed up for 1 month.The changes in clinical symptoms,quality of life KPS and dyspnea scores before and after treatment,fistula closure,and complications after stent placement were observed.Results Esophagotracheal fistula was more common in men,and it was most common in patients with malignant esophageal cancer.The most common clinical symptoms were cough in 52 cases(94.545％),chocking in 49 cases(89.091％),fever in 20 cases(36.364％)and dysphagia in 10 cases(18.182％).After one month of stent implantation treatment,the incidence of cough,chocking,fever,and dysphagia in patients was lower than that before treatment(P＜0.05).After one month of stent implantation treatment,55 patients had higher KPS scores than those before treatment and lower dyspnea scores than those before treatment(P＜0.05).Fistula closure after 1 month of stent implantation showed that 2 cases were in complete remission(3.636％),51 cases were in clinical complete remission(92.727％),2 cases were in partial remission(3.636％),and 0 case was ineffective(0.000％).Of the 55 patients,stent displacement occurred in 2 cases(3.636％)in typeⅠ stent;secretion retention was mild in 15 cases(27.273％),moderate in 30 cases(54.545％)and severe in 10 cases(18.182％);granulation tissue hyperplasia occurred in 7 cases(12.727％);tracheoesophageal fiistula recurred in 3 cases(5.455％).Conclusions Bronchoscopic stent placement can effectively seal esophagotracheal fistula and improve clinical symptoms.关键词
