iRoot BP Plus结合显微根尖手术治疗难治性根尖周炎的临床价值分析
Analysis on the Clinical Value of IRoot BP Plus Combined with Microapical Surgery in the Treatment of Refractory Periapical Radiculitis

Abstract
Objective To analyze the clinical effect of iRoot BP Plus combined with microsurgical surgery in the treatment of refractory periapical radiculitis.Methods 76 patients with refractory periapical radiculitis admitted to our hospital from June 2022 to June 2024 were selected and randomly divided into observation group and control group,with 38 cases in each group.All patients underwent microsurgical surgery,with the control group filled with silver amalgam and the observation group filled with iRoot BP Plus.The clinical efficacy,swelling and pain status,old periapical index(O-PAI),chewing function(chewing efficiency,bite force),gingival crevicular fluid inflammatory factors[interleukin-1(IL-1),interleukin-8(IL-8),tumor necrosis factor-α(TNF-α)]levels,and treatment satisfaction were compared between the two groups.Results No significant difference was found in the total effective rate of treatment between the two groups(P＞0.05).The swelling and pain rate of the observation group was 5.26％lower than 23.68％of the control group(P＜0.05).At 3 and 6 months after surgery,the O-PAI of the observation group was lower than that of the control group(P＜0.05).3 months after surgery,the chewing efficiency and bite force of the observation group were higher than those of the control group,while the IL-1,IL-8,and TNF-αlevels were lower than those of the control group(P＜0.05).The scores of post guidance,medical environment,rehabilitation status,and medical service in the observation group were higher than those in the control group(P＜0.05).Conclusions IRoot BP Plus combined with microsurgical surgery has significant effect in the treatment of patients with refractory periapical radiculitis,can reduce the swelling and pain,promotie the periapical tissue recovery,improve the chewing function,alleviate the gingival crevicular fluid inflammation symptoms,and improve the treatment satisfaction.关键词
