Abstract
Objective To observe the effect of free anterolateral thigh flap and superficial circumflex iliac artery perforator flap on repairing skin and soft tissue defects of limbs.Methods The clinical data of 90 patients who were hospitalized due to skin and soft tissue defects of limbs and underwent anterolateral thigh flap or superficial circumflex iliac artery perforator flap repair from January 2020 to January 2023 were retrospectively analyzed.According to different surgical methods,the patients were divided into 48 cases in free anterolateral thigh flap group(ALTF group)and 42 cases in superficial circumflex iliac artery perforator flap group(SCIP group).Surgical time,intraoperative blood loss and incidence rates of postoperative complications in donor and recipient areas,hemorheological indicators before and after surgery and affected limb and flap sensory recovery and cosmetic effect at 6 months after surgery were compared between the two groups.Results The surgical time and intraoperative blood loss in SCIP group were shorter or less than those in ALTF group(P＜0.05).There were no statistically significant differences in the incidence rates of postoperative complications between the two groups(P＞0.05).At 10 days after surgery,the plasma viscosity,whole blood viscosity,red blood cell aggregation index and deformation index were decreased in the two groups compared with those at 1 day after surgery(P＜0.05),but there were no differences between the two groups at 10 days after surgery(P＞0.05).Six months after surgery,the scores of the upper limb function rating scale were lower than those of the first day before surgery(P＜0.05),and the scores of the lower limb function scale were higher than those of the first day before surgery(P＜0.05).There were no statistically significant differences in the scores of upper limb function and flap sensory recovery between the two groups(P＞0.05).The rate of flap repair effect in SCIP group was higher than that in ALTF group(P＜0.05).Conclusion Both ALTF and SCIP flaps can be used as ideal donor areas for repairing the skin and soft tissue defects of limbs,and both can restore the wound sensation and have good cosmetic effect.Clinical selection can be made according to the actual situation of patients.关键词
