Massage with Peiyuan Tongbi Ointment for Knee Osteoarthritis Patients:A Randomized Controlled Trial

Abstract
Objective To evaluate the clinical effect of massgae with Peiyuan Tongbi Ointment(PYTBO)on knee osteoarthritis(KOA)patients.Methods Recruited were 140 KOA patients admitted to the Department of Traditional Chinese Medicine and Tuina,Fourth Clinical Medical College,Xinjiang Medical University between October 2023 and May 2024.Of these,136 patients met the inclusion criteria and were randomly divided into two groups,the experimental group received massage with PYTBO(69 cases),while the control group was treated with a placebo ointment(67 cases).Each session lasted for 20 min,once a day for 7 successive days.After two treatment cycles,comparisons were made between the two groups regarding knee joint pain assessed by the Visual Analog Scale(VAS),Western Ontario and McMaster Universities Osteoarthritis Index(WOMAC)scores,and improvements in patients'gait.This included changes in gait parameters,peak pressure loads and contact areas of static foot regions,as well as peak plantar load values and contact areas during dynamic support phases.Results After treatment,the VAS and WOMAC scores for knee joints in both groups significantly decreased compared to before treatment(P＜0.05)Additionally,the scores in the experimental group were lower than those in the control group(P＜0.05,P＜0.01).Both groups showed a reduction in double limb support time,while average walking speed,step length,and steps per minute increased post-treatment compared to pre-treatment(P＜0.05).The experimental group exhibited shorter foot axis angles and double limb support times,while showing higher average walking speed,step length,and steps per minute compared to the control group(P＜0.05,P＜0.01).In static testing,the peak pressure values and contact areas of the medial arch and medial heel in the experimental group were higher than those in the control group(P＜0.05,P＜0.01).Conversely,the peak pressure values and contact areas of the lateral arch,T1,and toes T2-T5 were lower in the experimental group than in the control group(P＜0.05,P＜0.01).In dynamic testing,the plantar load peak values and contact areas during support phases significantly increased for both groups post-treatment compared to pre-treatment(P＜0.05);the experimental group exhibited greater plantar load peak values and contact areas throughout the support phases than the control group(P＜0.05).Conclusions PYTBO significantly improves the gait parameters and plantar pressure distribution in KOA patients.This trial has completed registration at China Clinical Trial Registration Center(No.CHiCTR2300075829)关键词
