腔镜与开放保留乳头乳晕的全乳切除术联合即刻乳房重建的临床体会
Clinical experience of endoscopic versus open nipple and areola-sparing total mastectomy combined with immediate breast reconstruction

Abstract
Objective:To compare the operative efficacy of endoscopic and open nipple and areola-sparing total mastectomy(NSM)combined with immediate breast reconstruction(IBR)in treatment of breast cancer.Methods:A retrospective analysis was conducted on 85 breast cancer patients who underwent NSM with IBR using prosthetic reconstruction from Mar.2018 to Mar.2021.According to different surgical methods,patients were divided into open group(n=41)and laparoscopic group(n=44).General infor-mation,surgery-related indicators,complications,breast cosmetic effects,and satisfaction were compared between the two groups.Pa-tients were followed up regularly for 3 years,the progression-free survival of the two groups was compared and survival curves were plot-ted.Results:The laparoscopic group had longer operation time but less intraoperative blood loss,postoperative drainage,and shorter hos-pital stay than the open group(P<0.05).No significant difference was observed in the number of lymph node dissected between the two groups(P>0.05).There was no statistical difference in postoperative complications such as subcutaneous effusion,lymphedema,flap necrosis,infection,upper limb dysfunction,and prosthesis displacement between the laparoscopic group and the open group(P>0.05).The postoperative cosmetic effect score and subjective satisfaction of patients in the laparoscopic group were higher than those in the open group(P<0.05).There was no statistical difference in progression-free survival between the two groups after 3 years of follow-up(P>0.05).Conclusions:Laparoscopic nipple and areola-preserving radical resection with immediate prosthetic implantation and breast reconstruction is effective in treating breast cancer.It improves breast aesthetics,is safe and reliable,and is worthy of clinical application and popularization.关键词
