Abstract
Objective:To evaluate the application effect of bronchial blockers(BB)and double-lumen tubes(DLT)in lung isolation during thoracoscopic surgery.Methods:Randomized controlled trials on the application of BB and DLT in thoracoscopic sur-gery were searched in PubMed,Embase,Cochrane Library,Web of Science,China national knowledge infrastructure,Wanfang Data Knowledge Service Platform,Weipu Information Chinese Periodical Service Platform,and Chinese biomedical literature service system from inception to Jun.2024.RevMan 5.4 was used for research.Results:A total of 9 randomized controlled trials(678 patients)were included in the analysis,including 336 cases in the BB group and 342 cases in the DLT group.There were no statistically significant differences in terms of the quality of lung collapse(OR=0.98,95%CI=0.77~1.27,P=0.90),the rate of patients with device mal-position(OR=0.98,95%CI=0.44~2.18,P=0.96),and the rate of intraoperative hypoxemia(OR=0.71,95%CI=0.29~1.69,P=0.43).The time required for lung collapse in the BB group was significantly longer than that in the DLT group(MD=1.74,95%CI=0.28~3.91,P<0.01),the peak airway pressure in the BB group was significantly lower than that in the DLT group(MD=-4.32,95%CI=-4.64~-4.01,P=0.02),and the incidence of postoperative pharyngalgia in the BB group was significantly lower than that in the DLT group(OR=0.22,95%CI=0.14~0.36,P<0.01).Conclusions:BB and DLT are safe and feasible in lung isolation during thoracoscopic surgery,and BB has minimally invasive advantage of the respiratory tract.关键词
