超声造影鉴别诊断原发性肝癌和结直肠癌肝转移的临床研究
Clinical Study on Contrast-enhanced Ultrasound for Diagnosing Primary Liver Cancer and Colorectal Cancer Liver Metastasis

Abstract
Objective:By comparing the contrast-enhanced ultrasound(CEUS)features of primary liver cancer(PLC)with a lesion diameter of less than 5 cm and colorectal cancer liver metastasis(CCLM),this study aims to explore the clinical application value and innovation of CEUS in the differential diagnosis of these two types of malignant liver tumors.Methods:A total of 177 PLC and CCLM patients who underwent CEUS in our hospital from 2021 to 2022 were selected as study subjects,including 88 PLC patients and 89 CCLM patients.The CEUS examination was performed using sulfur hexafluoride contrast agent and a DD70 color Doppler ultrasound diagnostic instrument.The two-dimensional ultrasound results,blood flow signal scores,and CEUS-specific indicators,including time to enhancement,time to peak,peak enhancement intensity,enhancement rate,and 50％slope rate,were recorded and analyzed for both groups.A comprehensive diagnostic model was established,and its accuracy,sensitivity,specificity,and positive predictive ratewere evaluated.Results:The study found significant differences in the CEUS features between PLC and CCLM.PLC patients had significantly higher vascular signal evaluation scores,peak enhancement intensity,and peripheral enhancement in the arterial phase compared to CCLM patients(P＜0.05),with a positive predictive value of 78.1％.Conclusion:CEUS is an effective and innovative method for the differential diagnosis of small lesion diameter PLC and CCLM.Its high resolution and real-time dynamic observation of lesion blood flow perfusion changes provide important imaging reference information for clinical practice,aiding in early diagnosis and personalized treatment planning,and improving patient prognosis.关键词
