C-TIRADS超声指南联合超微血管成像技术对甲状腺结节性质的诊断价值
Diagnostic Value of C-TIRADS Guidelines Combined with Superb Micro-vas-cular Imaging in Characterization of Thyroid Nodules

Abstract
Objective:To evaluate the diagnostic value of Chinese guidelines for ultrasound malignant risk stratification of thyroid nodules(C-TIRADS)combined with superb micro-vascular imaging(SMI).Methods:A total of 116 patients with thyroid nodules were selected from the First Affiliated Hospital of Hebei North University after fine needle puncture or pathological diagnosis.All patients received two-dimensional gray scale ultrasound,color ultrasound,and SMI examination.Based on these examination results,C-TIRADS classification and SMI classification were performed.The diagnostic efficiency of C-TIRADS classification and SMI classification for malignant thyroid nodules was analyzed by receiver operating characteristic(ROC)curve.Results:141 thyroid nodules were detected in 116 patients,including 59 malignant nodules(41.84%)and 82 benign nodules(58.16%).C-TIRADS classification was used to classify thyroid nodules into malignant risk categories of 5,22,13,22,65 and 14 thyroid nodules in categories 2,3,4A,4B,4C and 5,respectively,with malignant rates of 0,0,7.69%,9.09%,66.15%and 92.85%.The classification of C-TIRADS was consistent with the pathological results(Kappa=0.625,P<0.001).There were 10,45,28,29,29 malignant risk categories of thyroid nodule ficat<0.001).There were 10,45,28,29,29 malignant risk categories of thyroid nodule fication of C-TIRADS was consistent relatively high consistency between SMI classification and pathological results(Kappa=0.606,P<0.001).The sensitivity,specificity and AUC(95%CI)of C-TIRADS in the diagnosis of malignant thyroid nodules were 94.92%,71.95%and 0.868(0.801~0.919),respectively,and SMI were 76.27%,84.15%and 0.860(0.792~0.913),respectively.The combined diagnosis of C-TIRADS and SMI was 94.92%,82.93%and 0.937(0.883~0.971),respectively.Both specificity and AUC were improved compared with single application,and the diagnostic efficiency of combined application was higher than that of single application(Z=4.103,3.501,P<0.001).Conclusion:C-TIRADS classification and SMI classification of thyroid nodules have high consistency with pathological diagnosis results.The combined diagnosis of C-TIRADS classification and SMI classification is helpful to improve the diagnostic efficiency of malignant thyroid nodules.关键词
