Color Doppler Ultrasound Diagnosis of Children with Abdominal Anaphylac-toid Purpura:a Study of False Positive Characteristics and Influencing Factors

Abstract
Objective:To investigate the characteristics and influencing factors of false positive in the diagnosis of abdominal anaphylactoid purpura by color Doppler ultrasound in children.Methods:A total of 120 children diagnosed with abdominal anaphylactoid purpura by color Doppler ultrasound in our hospital from January 2019 to January 2024 were selected and divided into a false-positive group(44 cases)and a true positive group(76 cases)according to the clinical efficacy.The clinical features and ultrasound signs of the two groups of children were compared and analyzed,and the risk factors affecting false positive diagnosis of color Doppler ultrasound were determined by multivariate Logistic regression analysis,and the prediction model was established.Meanwhile,44 children with the same period were collected as an external validation set.Results:There were significant differences between the two groups in age,total time of abdominal pain,vomiting,WBC,PLT,CRP,ESR and other clinical features,intestinal wall thickening degree,intestinal wall level,serous membrane and mesentery,intestinal wall echo and other ultrasonic signs(P＜0.05).Multivariate analysis identified 11 clinical factors closely related to false positives in color Doppler ultrasound diagnosis.The prediction model showed that when the prediction probability P=0.70,the prediction effect of the model is the best,and the C-index and AUC values were good.Conclusion:Age,total time of abdominal pain,vomiting,increased WBC,increased PLT,increased CRP,increased ESR,intestinal wall thickening degree,intestinal wall level,serous membrane and mesentery,and intestinal wall echo are the influencing factors for false positive diagnosis of abdominal purpura by color Doppler ultrasonography.The prediction model based on these factors can effectively predict the risk of false positives and provide a useful tool for improving the diagnostic accuracy.关键词
