颈部超声血流参数与突发性耳聋听力损失程度及转归不良的关系分析
Analysis of the Relationship between Neck Ultrasound Blood Flow Parameters and the Degrees of Hearing Loss and Poor Outcomes of Sudden Sensorineural Hearing Loss

Abstract
Objective:To explore the relationship between neck ultrasound blood flow parameters and the degree of hearing loss and poor outcomes with sudden sensorineural hearing loss(SSHL).Methods:The retrospective study included 155 patients diagnosed with SSHL between January 2021 and January 2024 as the disease group,neck ultrasound blood flow parameters were measured,including end-diastolic volume(EDV)and peak systolic velocity(PSV)of bilateral internal carotid,vertebral,and subclavicular arteries,compared with 148 healthy controls matched for age and sex,and the neck ultrasound blood flow parameters of patients with different degrees of hearing loss were compared.The disease group was divided into good outcomes group and poor outcomes group according to the disease outcomes,the clinical data of the 2 groups were compared,and the influencing factors of poor outcomes were analyzed by Logistic regression method.Pearson correlation analysis was used to evaluate the relationship between neck ultrasound blood flow parameters and the degree of hearing loss and poor outcomes in the disease group.Results:In the disease group,a total of 148 patients were analyzed after excluding 4 patients who were transferred to hospital and 3 patients who gave up treatment.Compared with the healthy control group,EDV and PSV in all assessed arteries were decreased in the disease group(P＜0.05).There were differences in EDV and PSV of all assessed arteries in patients with different degrees of hearing loss in the disease group(P＜0.05),moderate,severe and extremely severe patients were lower than mild patients(P＜0.05),severe and extremely severe patients were lower than moderate patients(P＜0.05),and extremely severe patients were lower than severe patients(P＜0.05).The incidence of poor outcomes in the disease group was 28.38％(42/148).Logistic regression analysis determined that type of hearing curve total hearing loss(Odds ratio(OR)=1.914,5％confidence interval(95％CI):1.28～2.854)were the independent risk factors for poor outcomes(P＜0.05),internal carotid artery EDV(OR=0.529,95％CI:0.376～0.745),vertebral artery EDV(OR=0.552,95％CI:0.405～0.752),subclavian artery EDV(OR=0.538,95％CI:0.356～0.814),internal carotid artery PSV(OR=0.555,95％CI:0.420～0.735),vertebral artery PSV(OR=0.569,95％CI:0.435～0.746),subclavian artery PSV(OR=0.560,95％CI:0.389～0.807)and hyperbaric oxygen therapy(OR=0.524,95％CI:0.410～0.669)were the protective factors for poor outcomes(P＜0.05).Pearson correlation analysis determined that EDV and PSV in all assessed arteries in the disease group were negatively correlated with the degree of hearing loss and poor outcomes(P＜0.05).Conclusion:Neck ultrasound blood flow parameters of patients with SSHL are closely related to the degree of hearing loss and poor outcomes,and the type of hearing curve total hearing loss is a risk factor for poor outcomes,and hyperbaric oxygen therapy is a protective factor for poor outcomes,these findings may be helpful for clinical evaluation and treatment planning of SSHL.关键词
