冠脉CTA与冠状动脉钙化评分在冠心病早期预警系统中的整合应用
Integrated Application of Coronary CTA and Coronary Artery Calcification Score in Early Warning System for Coronary Heart Disease

Abstract
Objective:The purpose of this study was to evaluate the clinical value of coronary computed tomography angiography(CTA)combined with coronary artery calcification score in the early warning of coronary heart disease.Methods:A total of 121 patients with suspected coronary heart disease who were admitted to hospital from June 2022 to January 2024 were selected as the study objects.All patients received coronary CTA examination with 64-row spiral CT and above,and the degree of coronary artery calcification was quantitatively assessed by Agatston scoring system.Using coronary angiography as the"gold standard"for the diagnosis of CHD,patients were divided into CHD positive group(n=82)and negative group(n=39)to compare the sensitivity,specificity and accuracy of coronaryartery CTA and coronary artery calcification score in the diagnosis of CHD.We analyzed the influencing factors of coronary heart disease through logistic regression analysis.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of multiple factors in coronary heart disease.Result:The sensitivity,specificity,accuracy and positive predictive value of CTA diagnosis were 96.34％,82.05％,91.74％and 91.86％respectively.The negative predictive value was 94.43％.The coronary artery calcification score in positive patients(206.39±33.87)was significantly higher than that in negative patients(156.75±22.69)(P＜0.05).Logistic regression analysis showed that coronary artery calcification score,smoking history,hypertension,hyperlipidemia and other factors were closely related to the occurrence of coronary heart disease.ROC curve analysis showed that the sensitivity and specificity of CTA in diagnosing CHD were 96.34％and 82.05％.The sensitivity and specificity of coronary artery calcification score were 69.51 and 92.31 respectively.The diagnostic value of combined diagnosis(AUC=0.966)was higher than that of single diagnosis(AUC=0.892,0.856).Conclusion:Coronary artery CTA combined with coronary artery calcification score has high clinical application value in the early warning of coronary heart disease,and can provide strong support for the early detection and treatment of coronary heart disease.关键词
