Abstract
Objective:To investigate the value of urinary tract CT Indicators for lower ureteral negative stones as a guide to extracorporeal shock wave lithotripsy(ESWL).Methods:Clinical cases of 590 patients who underwent ESWL for lower ureteral negative stones diagnosed by CT examination in our hospital from February 2020 to October 2023 were selected,and the patients were divided into the success group(n=501)and the failure group(n=89)according to the patients'treatment outcomes,and 65 patients each in the success group and the failure group were finally included after propensity 1∶1 matching.All patients underwent preoperative CT examination,and clinical data and CT parameters were collected to analyse the relationship between CT parameters and ESWL treatment outcomes.Results:The mean CT value of stones,the highest CT value,and the level of the largest diameter of stones in patients in the success group were significantly lower than those in the failure group(P＜0.05);and the distance from the centre of stones to the body surface in patients in the success group was significantly higher than that in the control group(P＜0.05);Logistic regression equation analysis showed that preoperative renal colic,preoperative tamsulosin,mean CT value of stones,maximum CT value,maximum diameter of stones,and distance from the centre of stones to the surface of the body were all relevant factors affecting the outcome of ESWL;ROC analysis showed that mean CT value of stones,maximum CT value,maximum diameter of stones,and distance from the centre of stones to the surface of the body could be used to predict the outcome of ESWL.The results of ROC analysis showed that the mean CT value,maximum CT diameter,maximum stone diameter,and distance from the centre of the stone to the body surface could all be used to predict ESWL treatment outcome,and the combination of the four was more effective,with an AUC of 0.889.Conclusion:CT can provide important parameters to predict the treatment outcome of ESWL for lower ureteral negative stones,in which the mean CT value of stones,the highest CT value,the maximum diameter of stones and the distance from the centre of stones to the body surface can be used together to predict the treatment outcome,which is a good guide for clinical ESWL treatment of lower ureteral negative stones.关键词
