Abstract
Background:Early onset preeclampsia(EOPE)is a serious pregnancy complication closely related to adverse maternal and perinatal outcomes.Color Doppler ultrasound is a non-invasive method for evaluating umbilical artery blood flow,but its value in predicting EOPE is not yet clear.Objective:The purpose is to explore the correlation between umbilical artery blood flow parameters and the severity of EOPE and pregnancy outcomes.Methods:Retrospective analysis of clinical data of 128 pregnant women with EOPE admitted to our hospital from January 2019 to December 2023,divided into severe group(n=57)and non severe group(n=71)according to the severity of EOPE.Another 120 healthy pregnant women who underwent prenatal examinations in our hospital during the same period were selected as the control group.Using color Doppler ultrasound to measure umbilical artery blood flow parameters,including pulsatile index(PI),resistance index(RI),and systolic to diastolic flow velocity ratio(S/D).Evaluate the relationship between these parameters and the severity of EOPE and adverse pregnancy outcomes through logistic regression analysis.Analyzing the dose-response relationship between umbilical artery blood flow parameters and adverse pregnancy outcomes using a restrictive cubic spline model combining spline function and logistic regression.Analyze and calculate the crude risk ratio(RR)between severe EOPE and perinatal adverse events using a univariate log binomial regression model.Results:The study found that the PI(1.26±0.24),RI(0.85±0.13),and S/D(2.71±0.48)of the case group were significantly higher than those of the control group(P<0.05),while the PI(1.36±0.21),RI(0.97±0.10),and S/D(2.89±0.34)of the severe group were significantly higher than those of the non severe group(P<0.05);PI,RI,S/D were positively correlated with the severity of EOPE(P<0.05),especially when PI≥ 1.1,RI≥0.7,and S/D ≥ 2.5,the sensitivity and specificity for predicting severe EOPE were 78%and 82%,respectively.There was a non-linear dose-response relationship(P<0.05)between umbilical artery blood flow parameters and adverse pregnancy outcomes.After refining the adverse time of perinatal infants,the probability of perinatal adverse events in severe EOPE pregnant women was higher than that in non severe EOPE pregnant women.Conclusion:Umbilical artery blood flow parameters PI,RI,and S/D are independent predictors of the severity of EOPE,which may help in early identification and risk stratification.关键词
