血清细胞因子检测对非结核分枝杆菌病的临床诊断意义及治疗分析
Clinical significance and treatment analysis of serum cytokine detection in nontuberculous mycobacteriosis

Abstract
Objective To explore the clinical significance of serum cytokine detection in nontuberculous mycobacteriosis(NTM)and its role in treatment,providing evidence for inflammation and immune status assessment,auxiliary diagnosis,long-term medication planning,and prognosis.Methods Serum levels of IL-1β,IL-6,IL-8,IL-10,IL-12P70,IFN-α,IFN-γ,and TNF-α were measured in 150 patients with NTM,50 patients with TB,and 50 healthy controls using flow cy-tometry.Results Five of the eight cytokines were significantly elevated in NTM patients,with IL-6 showing the strongest discriminative ability.Post-treatment IL-1β levels decreased significantly at 3 and 6 months compared with pre-treatment(P<0.05).Compared with that pre-treatment and 6 months post-treatment,the IL-6 concentration level increased signifi-cantly at 3 months after treatment(P<0.05),and the IL-6 concentration level in the resistant group was significantly high-er than that in the sensitive group(P<0.05);the IL-10 concentration level gradually increased with the longer treatment time.At the early stage of treatment,the IL-6 and IL-1β concentrations showed a positive correlation,IL-6 and IL-10 showed a negative correlation after 6 months of treatment;IL-6 and IFN-γ showed a good positive correlation at the early stage and 3 months of treatment.ROC curve analysis was performed in the NTM-resistant group,and the area under the curve of IL-1β com-bined with IL-6 was found to be the highest,AUC=0.819(95%CI:0.754-0.883).Conclusion IL-1 β,IL-6,IL-8,IFN-γ,and TNF-α can be used as auxiliary diagnostic markers of NTM infection.IL-6 and IFN-γ in patients with drug-resistant NTM will have the highest concentration level at 3 months of treatment.IL-6 and IFN-γ in patients with sensitive NTM will gradually decrease with the treatment cycle.IL-1 β combined with IL-6 can be used as an important indicator of the treatment prognosis of patients with NTM.Active control of NTM infection can reduce the excessive inflammatory response of cytokines in the human body.关键词
