Preoperative biliary drainage in hilar cholangiocarcinoma:current status and controversies
摘要
Abstract
Hilar cholangiocarcinoma(HCCA)is a highly malignant and common biliary tumor in the biliary system,with surgical resection being the most effective treatment.Most patients lack specific clinical symptoms in the early stages and of-ten present with biliary obstruction comorbid with jaundice due to tumor progression.Jaundice is frequently accompanied by complications such as cholangitis,liver dysfunction,coagulopathy,and delayed postoperative recovery.Therefore,preoper-ative biliary drainage(PBD)is commonly performed clinically to alleviate the negative impacts of jaundice,facilitate early surgical treatment,and promote postoperative recovery.However,controversies remain regarding the application of PBD,including procedural methods,post-drainage bilirubin control levels,and duration of drainage.This article reviews the cur-rent status and controversies of PBD in the preoperative management of HCCA to provide references for PBD treatment in HCCA patients.关键词
