Abstract
Objective To explore the clinical effect of multidisciplinary team(MDT)management combined with femoral head replacement in treating femoral neck fractures in elderly patients.Methods A total of 84 elderly patients with femoral neck fractures who underwent joint replacement surgery in our hospital from May 2022 to May 2024 were randomly allocated to a control group and a study group,with 42 patients in each.The control group received conventional orthopedic manage-ment,while the study group received MDT management in addition to the control group protocol.The two groups were compared in terms of preoperative and postoperative Visual Analogue Scale(VAS)scores,Harris Hip Score(HHS),Fugl-Meyer Assessment(FMA)scores,and serum inflammatory factor levels.Postoperative complication rates were also com-pared.Results Before surgery,there were no statistically significant differences in serum IL-6 and TNF-α levels between the two groups(P>0.05).At all postoperative time points,serum IL-6 and TNF-α levels in the study group were signifi-cantly lower than those in the control group(P<0.05);both groups showed significant reductions in IL-6 and TNF-α levels postoperatively(P<0.05).One hour before surgery,VAS scores were not significantly different between the groups(P>0.05);at all postoperative time points,the study group had significantly lower VAS scores than the control group(P<0.05),and both groups showed a significant downward trend in VAS scores over time(P<0.05).One hour before surger-y,FMA and HHS scores were not significantly different between the groups(P>0.05);both scores significantly increased postoperatively in both groups(P<0.05),and at 3 months after surgery,the study group had significantly higher FMA and HHS scores than the control group(P<0.05).The incidence of postoperative complications during hospitalization was 35.71%in the study group,significantly lower than 69.05%in the control group(χ2=9.355,P<0.05).Conclusion MDT management combined with femoral head replacement in elderly patients with femoral neck fractures yielded better clinical outcomes,reduced postoperative pain and inflammatory factor levels,improved hip joint function,and lowered the incidence of complications.关键词
