抗精子抗体联合抗苗勒管激素对卵巢低反应患者IVF-ET助孕结局的预测价值
Antisperm antibodies combined with anti-Mullerian hormone in predicting IVF-ET outcomes in patients with poor ovarian response

Abstract
Objective To investigate the predictive value of antisperm antibodies(AsAb)combined with anti-Mullerian hor-mone(AMH)on the outcomes of in vitro fertilization-embryo transfer(IVF-ET)in patients with poor ovarian response(POR),aiming to provide a reference for clinical treatment.Methods A total of 124 POR patients who underwent IVF-ET at the Reproductive Medicine Center of the Second Affiliated Hospital of Air Force Medical University from June 2022 to De-cember 2023 were selected for this study.Based on pregnancy outcomes,patients were assigned to a poor outcome group(n=45)and a good outcome group(n=79).Relevant data were collected,and multivariate logistic regression analysis was used to explore factors influencing IVF-ET outcomes in patients with POR.Receiver operating characteristic(ROC)curve analysis was employed to assess the predictive value of AsAb and AMH for IVF-ET outcomes.Results The good outcome group had lower AsAb levels and higher AMH levels compared to the poor outcome group,with statistically significant differences(P<0.05).The areas under the ROC curve(AUC)for AsAb,AMH,and their combined detection in predic-ting IVF-ET outcomes were 0.829(95%CI:0.779-0.874),0.764(95%CI:0.714-0.809),and 0.925(95%CI:0.875-0.970),respectively.The poor outcome group had a significantly longer duration of infertility and fewer antral folli-cle counts and retrieved oocytes compared to the good outcome group(P<0.05).Multivariate logistic regression analysis indicated that AsAb(OR=2.622,95%CI:1.700-4.044)and AMH(OR=2.912,95%CI:1.696-5.003)were factors influencing IVF-ET outcomes in POR patients(P<0.05).Conclusion High AsAb expression and low AMH levels are correlated with poor IVF-ET outcomes in patients with POR.Both can serve as predictive indicators for IVF-ET outcomes,with combined detection offering better predictive efficacy.关键词
