初诊原发性肝癌患者心理痛苦阈值及影响因素分析
Psychological distress threshold and influencing factors in newly diagnosed patients with primary liver cancer

Abstract
Objective To explore the psychological distress threshold and influencing factors in newly diagnosed patients with primary liver cancer,aiming to provide empirical evidence for guiding psychological rehabilitation in these patients.Methods A total of 343 newly diagnosed primary liver cancer patients were surveyed using the General Information Questionnaire,the Hospital Anxiety and Depression Scale(HADS),the Distress Thermometer(DT),and the Problem List(PL).A HADS score≥15 was used as the threshold for psychological distress.Receiver Operating Characteristic(ROC)curve analysis was performed to explore the threshold for psychological distress.Multivariate logistic regression analysis was conducted to iden-tify factors influencing psychological distress in these patients.Results Among the 343 patients newly diagnosed with pri-mary liver cancer,122(35.57%)had a HADS score≥15.ROC curve analysis determined that a DT score≥3 is the threshold for identifying psychological distress.The detection rate of psychological distress(DT score≥3)accounted for 46.36%(159/343).Multivariate logistic regression analysis revealed that factors such as treatment decision-making(OR=3.115;95%CI:1.151-8.427),interactions with partners(OR=10.141;95%CI:1.272-80.869),activities including bathing/dressing(OR=6.425;95%CI:2.994-11.422),pain(OR=5.118;95%CI:1.640-15.968),sleep(OR=3.594;95%CI:1.190-10.852),numbness in hands/feet(OR=8.950;95%CI:1.601-50.026),fear(OR=4.137;95%CI:1.132-7.422),and worry(OR=7.691;95%CI:2.666-22.182)were significant risk factors for psychologi-cal distress(P<0.05).Conclusion A DT score≥3 is the threshold for identifying psychological distress in primary liver cancer patients.The high incidence of psychological distress necessitates targeted and effective clinical psychological care in-terventions,considering factors such as treatment decision-making,interactions with partner,and family health issues.关键词
