Consistency analysis of glucose metrics between continuous glucose monitoring and self-monitoring of blood glucose in children with type 1 diabetes mellitus

Abstract
Objective To evaluate the consistency of glucose metrics obtained from self-monitoring of blood glucose(SMBG)and continuous glucose monitoring(CGM)in children with type 1 diabetes mellitus(T1 DM).Methods We retrospectively analyzed CGM and SMBG data from T1 DM children treated at Children's Hospital of Chongqing Medical University between November 2019 and September 2021.Consistency between CGM-and SMBG-derived glucose metrics was assessed using the Wilcoxon signed-rank test,Kendall's W coefficient,and Bland-Altman plots.Results Thirty-six paired SMBG and CGM datasets were included.No significant differences were found between the two methods in calculating time in range(TIR),time above range(TAR),time below range(TBR),mean blood glucose(MBG),or glucose variability(GV)of glucose(P>0.05).However,a significant difference was observed in largest amplitude of glucose excursions(LAGE)(P<0.05).Kendall's W analysis showed strong consistency for TIR and TBR(Kendall's W=0.77 and 0.73,respectively),and highly strong consistency for TAR,MBG,and GV(Kendall's W=0.83,0.89,and 0.90,respectively).Bland-Altman plots demonstrated good overall agreement between SMBG and CGM for TIR,TAR,TBR,MBG,and GV.Conclusion SMBG and CGM show high consistency in assessing glucose metrics including TIR,TAR,and TBR in children with T1 DM.For pediatric patients without access to CGM,SMBG can serve as a reliable alternative monitoring method.关键词
