2型糖尿病视网膜病变患者外周血单个核细胞FOXO3a mRNA、miR-1271-5p表达与疾病相关性分析
Correlation between the expression of FOXO3a mRNA and miR-1271-5p in peripheral blood mononuclear cell of patients with type 2 diabetic retinopathy and the occurrence and progression of disease

Abstract
Objective To investigate the relationship between the expression levels of fork head box O3a(FOXO3a)messenger RNA(mRNA)and micro RNA-1271-5p(miR-1271-5p)in peripheral blood mononuclear cells(PBMC)of patients with type 2 diabetic retinopathy(T2DR)and the occurrence and progression of T2DR.Methods The prospertive cases-control study.A total of 87 T2DR patients admitted to the Zhangjiakou Fourth Hospital from September 2021 to May 2023 were selected as T2DR group,according to the results of fluorescein fundus angiography in T2DR pa-tients,they were divided into 46 cases in non-proliferating T2DR(NPDR)group and 41 cases in proliferating T2DR(PDR)group.In addition,90 patients with simple type 2 diabetes mellitus(T2DM)were included as T2DM group and 85 healthy subjects as control group.The diabetes course of patients in T2DM group and T2DR group,sex,age,glycated he-moglobin(HbA1c),fasting blood glucose(FBG)levels of control group,T2DM group and T2DR group were collected and compared;the expression levels of FOXO3a mRNA and miR-1271-5p in PBMC were detected and compared by real-time fluorescence quantitative PCR;the correlation between FOXO3a mRNA and miR-1271-5p expression levels in PBMC of T2DR patients was analyzed by Pearson method;multivariate Logistic regression analysis of factors affecting the occur-rence of T2DR in T2DM patients;the predictive value of FOXO3a mRNA and miR-1271-5p expression levels in PBMC for T2DR and PDR were analyzed by subject working characteristic curves.Results Compared with T2DM group,the diabe-tes course in T2DR group was significantly increased(P<0.05);the HbA1c,FBG levels,FOXO3a mRNA expression level of PBMC in control group,T2DM group and T2DR group were increased successively,while the expression levels of miR-1271-5p were decreased successively(P<0.05);compared with NPDR group,the FOXO3a mRNA expression level of PBMC in PDR group was significantly increased,and the miR-1271-5p expression level was significantly decreased(P<0.05);the expression levels of FOXO3a mRNA and miR-1271-5p in PBMC of T2DR patients were negatively corre-lated(P<0.05);the diabetes course and FOXO3a mRNA were risk factors for occurrence of T2DR in T2DM patients,and miR-1271-5p was protective factor for occurrence of T2DR in T2DM patients(P<0.05);the area under curve(AUC)of FOXO3a mRNA,miR-1271-5p,and their combination of PBMC in predicting the occurrence of T2DR in T2DM pa-tients were 0.880,0.691,and 0.954,respectively;the AUC of FOXO3a mRNA,miR-1271-5p,and their combination of PBMC in predicting the occurrence of PDR in T2DR patients were 0.884,0.909,and 0.955,respectively;compared with the AUC predicted by FOXO3a mRNA and miR-1271-5p separately,the AUC predicted by the combination of FoxO3A mrna and miR-1271-5p was higher(P<0.05).Conclusions The expression level of FOXO3a mRNA and miR-1271-5p in PBMC of T2DR patients was higher,and the expression level of miR-1271-5p was lower,both of which were correlated with the occurrence and progression of T2DR.The combination of FOXO3a mRNA and miR-1271-5p had high predictive value for the occurrence of T2DR and PDR.关键词
