Abstract
Objective:To analyze the correlation between the lesion degree and serum hemoglobin A1c(HbA1c),interleukin-6(IL-6)and cystatin C(CysC)in elderly patients with diabetic nephropathy(DKD).Methods:A total of 80 elderly patients with DKD treated in Jinqiu Hospital of Liaoning Province from December 2021 to June 2023 were selected as the experimental group,78 patients with simple elderly diabetes mellitus(DM)treated during the same period were selected as the control group,and 76 elderly healthy subjects were selected as the healthy group.Serum levels of HbA1c,IL-6,CysC and glomerular filtration rate(GFR)were determined.According to GFR,the test groups were divided into stage a I group(n=10),a stage Ⅱ group(n=18),a stage Ⅲ group(n=19),a stageⅣ group(n=13)and a stage Ⅳ group(n=20).The levels of serum indexes in different groups were determined.The correlation between serum levels of HbA1c,IL-6 and CysC and the degree of DKD lesions were further analyzed.Results:The levels of serum HbA1c,IL-6 and CysC in test groups were higher than those in control group and healthy group,and GFR was lower than those in control group and healthy group,with statistical significance(P<0.05).Multivariate Logistic regression analysis showed that the serum levels of HbA1c,IL-6 and CysC were increased,and the level of GFR was decreased(P<0.05).The levels of serum HbA1c,IL-6 and CysC in stage V group were higher than those in stage I,stage Ⅱ,stage Ⅲ and stage Ⅳ,and the GFR were lower than that in stage I,stage Ⅱ,stage Ⅲ and stage Ⅳ,with statistical significance(P<0.05).Pearson correlation analysis showed that serum levels of HbA1c,IL-6 and CysC were negatively correlated with GFR(P<0.05).Conclusion:Serum HbA1c,IL-6,CysC and GFR are related to the occurrence of DKD in the elderly.The degree of DKD lesions in the elderly is negatively correlated with serum HbA1c,IL-6 and CysC levels,which can provide reference for clinical evaluation and individualized treatment.关键词
