Construction and Preliminary Application of a Discharge Plan for Premature Infants with Broncho-pulmonary Dysplasia Based on IMB Model

Abstract
Objective To construct a discharge plan for premature infants with bronchopulmonary dyspla-sia(BPD)based on the information-motivation-behavioral skills(IMB)model and preliminarily evaluate its application.Methods Guided by an evidence-based approach and the IMB theoretical model,the dis-charge plan for premature BPD infants was initially designed around informational support,motivational intervention and behavioral skills.The final plan was validated through panel discussions.The study in-cluded 40 premature BPD infants and their caregivers admitted to the neonatology ward at a Grade Ⅲ-A tertiary hospital in Nantong from January to December 2023.They were divided into two groups according to their admission order.The control group(n=15)was given standard discharge care,whereas the observa-tion group(n=25)was given IMB-based discharge care.Maternal self-efficacy scores and infant re-admission rates within three months were compared between groups.Results A total of 12 articles were in-cluded,including 4 guidelines,6 expert consensuses,1 summary of evidence and 1 systematic review.The final plan spanned three stages of premature BPD infants:admission,hospitalization and peri-discharge.It provided stage-specific guidance to caregivers on informational support,motivational intervention and behavioral skills,with an authoritative coeffi-cient of 0.79.According to the validation results,the observation group demonstrated significantly higher maternal self-efficacy scores at discharge and first outpatient follow-up compared to the control group(P<0.05);the observation group demonstrated significantly lower premature infant re-admission rates within three months compared to the control group(P<0.05).Conclusion The IMB-based discharge plan for pre-mature BPD infants demonstrates scientific validity and feasibility.It effectively enhances maternal self-efficacy and reduces infant re-admission rates within 3 months after discharge.关键词
