基于结构方程模型的慢性心衰患者服药依从性影响因素分析
Analysis on Factors Influencing Medication Adherence in Chronic Heart Failure Patients Based on Structural Equation Model

Abstract
Objective To investigate medication adherence and its influencing factors in chronic heart failure(CHF)patients,providing references for developing effective interventions.Methods The study selected 300 CHF patients who achieved stable disease after treatment and were discharged from the Affiliated Hospital of Jinggangshan University between May 2023 and May 2024 by convenience sampling.Data were collected from these patients using the General Information Questionnaire,Morisky Medication Adherence Scale(MMAS),Fear of Intimacy with Helping Professionals Scale(FIS-HP),European Heart Failure Self-care Behaviour Scale(EHFScBS)and Comprehensive Score for Financial Toxicity(COST).Path analysis was conducted using the structural equation model.Results The MMAS score of CHF patients was(6.51±1.87)points.Factors influenc-ing medication adherence included the number of chronic comorbidities and medication types(P<0.05).Healthcare team,patient self-care ability and financial burden were positively correlated with medication ad-herence(P<0.05).The structural equation model demonstrated good fit(χ2/df=1.127).Path analysis revealed that self-care ability,healthcare team,number of chronic comorbidities,medication types and quantities,and economic burden exerted direct effects on medication adherence(path coefficients:0.376,5.442,-0.150,-0.299 and-4.689;all P<0.05);self-care ability and economic burden exerted indirect effects on medication adherence(path coefficients:0.151 and 0.300;both P<0.05);self-care ability and healthcare team acted as par-tial mediators,accounting for 37.38%and 46.15%of the total effects respectively.Conclusion Patient self-care ability,healthcare team,number of chronic comorbidities,medication types and economic burden directly influence medication adherence in CHF pa-tients.Individualized regimens should be developed based on patient-specific conditions to enhance medica-tion adherence in clinical practice.关键词
