Abstract
Objective:To investigate the clinical efficacy of internal fixation with pronator quadratus preservation in the treatment of patients with distal radius fractures.Methods:A retrospective analysis was conducted on the data of 100 patients with distal radius fractures admitted to the Beijing Huairou Hospital from March 2021 to July 2023,and they were divided into a control group and a study group according to the different surgical methods,with 50 cases in each group.The control group underwent internal fixation with incision of pronator quadratus,while the study group underwent internal fixation with pronator quadratus preservation.The perioperative indicators,pain level,wrist joint range of motion,grip strength recovery,and wrist joint function were compared between the two groups.Results:There were no statistically significant difference in the surgical time and intraoperative bleeding between the two groups(P>0.05);the hospitalization time and fracture healing time of the study group were shorter than those of the control group,the differences were statistically significant(P<0.05).At 3 days and 2 months after surgery,the scores of Pain Visual Analogue Scale in both groups were lower than preoperative,and at 3 days after surgery,the study group were lower than the control group,the difference were statistically significant(P<0.05);but at 2 months after surgery,there was no significant difference in VAS scores between the two groups(P>0.05).At 2 and 12 months after surgery,the range of motion for palmar flexion,dorsiflexion,radial deviation,and ulnar deviation of the wrist joints in both groups were bigger than preoperative,and at 2 months after surgery,the study group were bigger than those of the control group,the differences were statistically significant(P<0.05);but at 12 months after surgery,there were no significant differences in the range of motion for palmar flexion,dorsiflexion,radial deviation between the two groups(P>0.05).At 2 and 12 months after surgery,the percentage of affected grip strength to healthy grip strength were higher than preoperative,while the Gartland Welley score were lower than preoperative,and at 2 months after surgery,the percentage of affected grip strength to healthy grip strength of the study group was higher than that of the control group,while the Gartland-Welley score was lower than that of the control group,the differences were statistically significant(P<0.05);but at 12 months after surgery,there were no significant differences in the percentage of affected grip strength to healthy grip strength and Gartland-Welley score between the two groups(P>0.05).Conclusion:The treatment of internal fixation with pronator quadratus preservation through the musle-bone channel for patients with distal radius fractures can reduce postoperative pain,shorten hospitalization time,accelerate fracture healing,and promote the recovery of wrist joint mobility and function.关键词
