综合监护室重症病人肢体活动分级护理路径的构建及应用
Construction and application of a graded limb activity nursing path of critically ill patients in comprehensive intensive care unit

Abstract
Objective:To construct a graded limb activity nursing path of critically ill patients in comprehensive intensive care unit and to explore its preliminary application effect.Methods:Based on the concept of early activities oriented towards goals and a literature review,expert consultations were carried out to construct a graded limb activities nursing pathway for critically ill patients.From February to March 2024,this pathway was preliminarily applied among 21 critically ill patients(observation group)admitted to the general intensive care unit(ICU)of a tertiary grade A hospital in Beijing.It was then compared with 18 patients(control group)who received routine nursing care from December 2023 to January 2024,so as to evaluate the safety,feasibility and effectiveness of the pathway.Results:The attendance rate of the expert meetings and the effective questionnaire recovery rate of the expert consultations were both 100%.The expert authority coefficient was 0.85.The importance scores and feasibility scores of each item were all>3.5.The coefficient of variation was all<0.25,And the Kendall's coefficient of concordance was 0.564(P<0.001).The constructed graded limb activity nursing path for critically ill patients included 3 first-level items,11 second-level items,and 42 third-level items.The preliminary application results showed that no adverse events occurred during the implementation of the pathway.And the difference in the sum of the left and right handgrip strengths before and after the implementation of the pathway in the observation group was higher than that in the control group(P=0.005).Conclusions:The constructed graded limb activity nursing path for critically ill patients in the comprehensive intensive care unit has good scientificity and reliability,and is highly clinically feasible and safe.It is necessary to expand the sample size to further verify its clinical application effect.关键词
