Evaluation of lymph node metastasis prediction models for endometrial cancer in a mul-ticenter real-world database from China

Abstract
Objective:Numerous models are available to predict lymph node metastasis(LNM)for endometrial cancer(EC),but which is the most accurate remains unclear.We aimed to evaluate prediction models'performance in a multicenter real-world database.Meth-ods:A retrospective analysis was conducted on a large cohort of 21285 EC patients from the Chinese Endometrial Cancer Consortium(CECC)database.We evaluated the discriminatory a-bility of the 11 models(four preoperatively and seven postoperatively)by comparing their area under the receiver operating characteristic curve(AUC),accuracy(ACC),sensitivity,specific-ity,positive likelihood ratio(PLR),negative likelihood ratio(NLR),positive predictive value(PPV),negative predictive value(NPV),and the number of patients classified as low-risk for LNM.Results:Among the study population,8.0％of patients(1713 out of 21285)have LNM.Two preoperative and two postoperative models exhibited AUC greater than 0.70,ten models a-chieved a false negative(FN)rate of less than 5％,and eight models could assign more than 50％of patients to low-risk.Conclusions:The KGOG model and Mayo-modified criteria are better preoperative tools to guide decision-making regarding lymphadenectomy in EC.Without lymphadenectomy,the Sofiane et al.score or Dong et al.nomogram could be employed to deter-mine the need for a secondary lymphadenectomy.关键词
