胎儿心脏定量技术评价妊娠期高血压疾病患者胎儿心脏和功能的初步研究
A preliminary study on the evaluation of fetal heart morphology and function in hyper-tensive disorders of pregnancy by fetal heart quantitative technology

Abstract
Objective:To explore the fetal heart morphology and function in hyperten-sive disorders of pregnancy(HDP)by fetal heart quantification technology(fetal HQ).Meth-od:A total of 92 fetuses who underwent prenatal ultrasound examination at the Second Affiliated Hospital of Wenzhou Medical University and Yuying Children's Hospital between January 2023 and August 2023 were included.Among them,28 fetuses with gestational hypertension were se-lected as the HDP group,and 64 healthy single fetuses were selected as the control group.Standard four chamber dynamic images were collected,with a frame rate of>80Hz and a stor-age time of>3seconds.Enter the fetal HQ analysis software and measure the length(the widest distance between the epicardium of the four-chamber view)and transverse diameter(widest dis-tance between the epicardium in the four chamber view)of the heart on static images of the end diastolic four chamber view.The M-mode determines the cardiac cycle,and the contour of the ventricular intima is determined at the end systolic and end diastolic stages of the left and right ventricles,respectively.Result:The fetal HQ software quickly obtained various quantitative pa-rameters,including the global spherical index(GSI)of the fetal heart,global longitudinal strain (GLS)of the left and right ventricles,ejection fraction(EF)of the left ventricle,fractional area change(FAC),fractional shortening(FS),and sphericity index index(SI)of the 24 segments of the left and right ventricles.Comparative analysis was conducted on two sets of quantitative parameters.Among them,the GSI,GLS,and FAC of the left and right ventricles in the HDP group were significantly reduced compared to the control group(P<0.05).There was no statis-tically significant difference in the comparison of left ventricular EF and 24 segments of SI be-tween the two groups of fetuses(P>0.05).Conclusion:The cardiac contraction function of HDP fetal heart is reduced,and the overall morphology of the HDP group fetal heart is more spherical compared to the normal group,and there is no significant change in the local shape of the left and right ventricles.Fetal HQ can provide multiple parameters for simple and rapid quantitative evaluation of fetal heart shape and function,providing important information for clinical evaluation of fetal heart function in gestational hypertension.关键词
