Abstract
Objective To analyze the efficacy of Guben Kechuan Granules combined with Umeclidinium and Vilanterol in the stable phase of Chronic Obstructive Pulmonary Disease(COPD)with Lung-Kidney Qi Deficiency and Yang Deficiency Syndrome. Methods Ninety-four patients with stable-phase COPD(with Lung-Kidney Qi Deficiency and Yang Deficiency Syndrome)treated at Nanyang Oilfield General Hospital from March 2021 to March 2023 were selected and randomly divided into two groups. The control group(47 cases)received Umeclidinium and Vilanterol treatment, while the combination group(47 cases)also received Guben Kechuan Granules. The groups were compared in terms of traditional Chinese medicine syndrome scores, modified Medical Research Council(mMRC)dyspnea scale scores,COPD Assessment Test(CAT)scores, clinical efficacy, lung function indicators, blood gas analysis indicators, exercise endurance, proportion of peripheral blood helper T cells 17(Th17),regulatory T cells(Treg),Th17/Treg ratio, adverse reactions, and the number of acute exacerbations. Results After treatment, the traditional Chinese medicine syndrome scores,mMRC scores, and CAT scores decreased in both groups, with lower scores in the combination group compared to the control group, showing significant statistical differences(P<0.05).The overall efficacy of the combination group was superior, with significant statistical differences between the groups(P<0.05).Post-treatment,forced expiratory volume in one second(FEV1),peak expiratory flow(PEF),and forced vital capacity(FVC)increased in both groups, with higher levels observed in the combination group compared to the control group, showing significant statistical differences(P<0.05).Post-treatment,partial pressure of oxygen(PaO2)increased and partial pressure of carbon dioxide(PaCO2)decreased in both groups, with longer 6-minute walking distances(6MWD).The combination group had higher PaO2,lower PaCO2,and longer 6MWD than the control group, with significant statistical differences(P<0.05).Post-treatment,Th17 and Th17/Treg levels decreased, and Treg levels increased in both groups, with the combination group exhibiting lower Th17 and Th17/Treg levels and higher Treg levels than the control group, showing significant statistical differences(P<0.05).The incidence of adverse reactions showed no significant statistical difference between the groups(P>0.05).The combination group had fewer acute exacerbations than the control group, with a significant statistical difference(P<0.05).Conclusion For patients with stable-phase COPD(Lung-Kidney Qi Deficiency and Yang Deficiency Syndrome),treatment with Guben Kechuan Granules combined with Umeclidinium and Vilanterol can alleviate symptoms, enhance efficacy, improve lung function and blood gas indicators, increase exercise endurance, and is safe and reliable, making it worthy of clinical application.关键词
