基于药物利用研究指标评价质子泵抑制剂的DRGs费用及合理性
Evaluation of DRGs Cost and Rationality of Proton Pump Inhibitors Based on the Drug Utilization Research Indexes

Abstract
Objective To evaluate the cost and rationality of diagnosis-related group(DRGs)for proton pump inhibitors(PPIs)based on drug utilization research(DUR)indexes,and to provide a reference for rational drug use in medical institutions.Methods Based on six DRGs groups(groups Ⅰ-Ⅵ),relevant information on the drug costs of PPIs used by patients with digestive system diseases admitted to a tertiary general hospital in Dali Bai Autonomous Prefecture,Yunnan Province from January to June 2022(before intervention)and from July to December 2022(after intervention)was collected.The DUR indexes[including defined daily dose system(DDDs),drug utilization index(DUI),PPIs usage rate,and antibiotics use density(AUD)]and the rationality of medical orders in each group were evaluated.Results Compared to those before intervention,the average medication cost of PPIs in the groups Ⅲ,Ⅳ,Ⅵ decreased after intervention,while that in the groups Ⅰ,Ⅱ,Ⅴ groups increased after intervention.In terms of average drug proportion,compared to those before intervention,the average drug proportion in groupsⅣ,Ⅵ decreased after intervention,while that in groups Ⅰ,Ⅱ,Ⅲ,Ⅴ increased after intervention.Compared to those before intervention,the DUI in groups Ⅳ,Ⅵ decreased after intervention,while that in groups Ⅰ,Ⅱ,Ⅲ,Ⅴ increased after intervention.The usage rate of PPIs in group Ⅲ decreased from 90.32%to 64.29%.Before and after intervention,the AUD of PPIs in groupsⅠ,Ⅱ,Ⅴ was higher than that in groups Ⅲ,Ⅳ,Ⅵ.Compared with those before intervention,the prescription qualification rates in groups Ⅰ-Ⅵ after intervention were higher(80.00%,53.33%,92.86%,84.62%,78.79%,and 53.62%,respectively).Conclusion Evaluating the DRGs cost and medication rationality of PPIs based on DUR indexes is scientific and feasible.关键词
