Abstract
OBJECTIVE To analyze the acupoint selection patterns in acupuncture treatment of optic atrophy using the Ancient and Modern Medical Case Cloud Platform.METHODS Relevant literature on acupuncture for optic atrophy was retrieved from CNKI,VIP,Wanfang,CBM,Chinese Science and Technology Journal Database,Embase,Cochrane Library,and PubMed from their inception to May 31st,2024.Studies were screened based on inclusion and exclusion criteria,and a prescription database was established with standardized acupoint nomenclature.The data mining module of the Ancient and Modern Medical Case Cloud Platform V2.3.9 was used to conduct frequency analysis,meridian distribution analysis,association rule analysis,community analysis,complex network analysis,and cluster analysis.RESULTS(1)General Information:A total of 3,321 studies were retrieved,with 326 meeting the inclusion criteria,yielding 210 valid acupuncture prescriptions.(2)Acupoint frequency:The top five frequently used acupoints were Jingming(BL-1,148 times,70.48%),Fengchi(GB-20,136 times,64.76%),Qiuhou(EX-HN7,120 times,57.14%),Chengqi(ST-1,115 times,54.76%),and Cuanzhu(BL-2,114 times,54.29%).(3)Meridian attribution:The most frequently involved meridians were the foot Taiyang bladder meridian(559 times,25.58%),foot Shaoyang gallbladder meridian(339 times,15.51%),extraordinary points(306 times,14.00%),foot Taiyin spleen meridian(237 times,10.85%),and foot Yangming stomach meridian(185 times,8.47%).(4)Acupoint location:Acupoints were primarily located in the head,face,and neck(1,282 times,58.67%),followed by the lower limbs(525 times,24.03%),upper limbs(188 times,8.60%),lumbar and back(162 times,7.41%),and chest and abdomen(28 times,1.28%).(5)Special acupoints:The most commonly used special acupoints were intersection points(294 times,25.90%),Yuan-primary points(247 times,22.60%),five-Shu points(201 times,18.39%),back-Shu points(140 times,12.81%),and Luo-connecting points(108 times,9.88%).(6)Acupoint pairing association rules:The top five most strongly associated acupoint pairs were Qiuhou-Jingming,Chengqi-Jingming,Sanyinjiao(SP-6)-Jingming,Sanyinjiao-Zusanli(ST-36),and Taichong(LR-3)-Jingming.(7)Complex network analysis:The core acupoint combination for treating optic atrophy consisted of Jingming,Qiuhou,Chengqi,Fengchi,and Zusanli.(8)Needling techniques and retention time:Among the included literature,the most commonly used technique was reinforcing and reducing manipulation based on syndrome differentiation(67 articles),and the most common needle retention time was 30 minutes(150 articles).(9)TCM syndrome differentiation:The top five TCM patterns were liver Qi stagnation(677 cases,41.89%),Qi stagnation and blood stasis(322 cases,19.93%),deficiency of liver and kidney(112 cases,6.93%),deficiency of both Qi and blood(111 cases,6.87%),and Yin deficiency of liver and kidney(108 cases,6.68%).CONCLUSIONS Acupuncture treatment for optic atrophy primarily involves the foot Taiyang bladder meridian,with an acupoint selection strategy that follows a distal-proximal combination principle.The approach prioritizes local acupoints,especially in the head and face region,with an emphasis on specific-category acupoints.The core acupoints include Jingming,Qiuhou,Chengqi,Fengchi,and Zusanli.Additional acupoints should be selected based on syndrome differentiation to optimize therapeutic efficacy.关键词
