Clinical Value of Peripheral Blood NLR,CAR and SF Levels in Evaluating the Disease Severity and Prognosis of Children with Mycoplasma Pneumoniae Pneumonia

Abstract
Objective To analyze the clinical value of peripheral blood neutrophil/lymphocyte ratio(NLR),C-reactive protein/albumin ratio(CAR),and serum ferritin(SF)levels in evaluating the disease severity and prognosis of mycoplasma pneumoniae pneumonia(MPP)in children.Methods Data from 104 children with MPP treated at the Department of Pediatrics,Honghui Hospital Affiliated to Xi'an Jiaotong University,were retrospectively analyzed from January 2021 to March 2024,and the children were included in the MPP group.Another 63 healthy children undergoing physical examinations were selected as the healthy control group.The levels of peripheral blood NLR,CAR,and SF were compared between groups.According to the clinical pulmonary infection score(CPIS),the children with MPP were divided into a mild group(n=56)and a moderate-to-severe group(n=48).The levels of peripheral blood NLR,CAR,SF,and the Acute Physiology and Chronic Health Evaluation Ⅱ(APACHE Ⅱ)score were compared between the mild and moderate-to-severe groups.Pearson correlation analysis was used to analyze the correlation between peripheral blood NLR,CAR,SF levels,and the APACHE Ⅱ score.By means of efficacy criteria,the children were classified into a poor prognosis group(n=69)and a good prognosis group(n=35),and the changes in the above indicators were compared between the two groups.Receiver operating characteristic(ROC)curves were drawn to analyze the diagnostic efficiency of peripheral blood NLR,CAR,SF,and their combination for predicting poor prognosis in children with MPP.Results The levels of NLR,CAR,and SF in peripheral blood in the MPP group were higher than those in the healthy control group,with statistical significance(P<0.05).The levels of peripheral blood NLR,CAR,SF,and the APACHE Ⅱ score in the moderate-to-severe group were higher than those in the mild group,with statistical significance(P<0.05).The peripheral blood NLR,CAR,SF levels,and APACHE Ⅱ score were higher in the poor prognosis group than in the good prognosis group,with statistical significance(P<0.05).Pearson correlation analysis showed that the levels of NLR,CAR,and SF in peripheral blood of children with MPP were positively correlated with the APACHE Ⅱ score(r=0.813,0.798,0.808,P<0.001).ROC curve analysis revealed that the AUCs for peripheral blood NLR,CAR,SF,and their combination were 0.882,0.883,0.854,and 0.897,respectively.The sensitivities for predicting poor prognosis were 82.86%,80.00%,77.14%,and 85.71%,and the specificities were 92.75%,92.75%,94.20%,and 91.30%,respectively.Conclusion The levels of NLR,CAR,and SF in peripheral blood of children with MPP are significantly higher than those in healthy children.With the aggravation of the disease,the levels of these indicators increase and are positively correlated with disease severity.The combination of these three indicators has high diagnostic efficiency for predicting poor prognosis in children with MPP.关键词
