Relationship of Serum SDC1 and FGFR3 Levels with Glycolipid Metabolism and Retinopathy in Patients with Type 2 Diabetes

Abstract
Objective To investigate the relationship between serum levels of syndecan-1(SDC1),fibroblast growth factor receptor 3(FGFR3)and glycolipid metabolism,as well as diabetic retinopathy(DR)in type 2 diabetes mellitus(T2DM)patients.Methods Sixty-six patients with T2DM admitted to the First Affiliated Hospital of Air Force Medical University from May 2019 to May 2020 were included as the T2DM group.Additionally,104 T2DM patients with DR were divided into non-proliferative DR group(n=60)and proliferative DR group(n=54)based on disease staging.Another 50 healthy volunteers undergoing physical examination during the same period were included as the control group.General data,serum SDC1,FGFR3,and glycolipid metabolism indicators[lipid metabolism:total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C);glucose metabolism:fasting plasma glucose(FPG),fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR),and glycosylated hemoglobin(HbA1c)]were compared across the groups.Pearson correlation was used to analyze the relationship between SDC1,FGFR3,and glycolipid metabolism in T2DM patients.ROC curve analysis was performed to evaluate the predictive value of SDC1 and FGFR3 for DR,and Logistic regression identified factors associated with DR.Results Levels of SDC1,FGFR3,TC,LDL-C,FPG,FINS,HOMA-IR,and HbA1c increased significantly in the order:proliferative DR group>non-proliferative DR group>T2DM group>control group(P<0.05).HDL-C levels showed an opposite trend,being lowest in the proliferative DR group and highest in the control group(P<0.05).SDC1 and FGFR3 were positively correlated with TC,LDL-C,FPG,FINS,HOMA-IR,and HbA1c,but negatively correlated with HDL-C(P<0.05).ROC analysis showed that the AUC values for predicting DR using SDC1 and FGFR3 were 0.748(95%CI:0.698-0.798)and 0.756(95%CI:0.706-0.806),respectively.The combined AUC was 0.889(95%CI:0.839-0.939).Multivariate analysis revealed that SDC1≥45.63 ng/mL(OR=3.904),FGFR3≥3.52 μg/L(OR=4.367),HbA1c≥8.22%(OR=3.367),HOMA-IR≥4.62(OR=1.923),and disease duration≥7.25 years(OR=2.423)were independent risk factors for DR(P<0.05).Conclusion Serum SDC1 and FGFR3 levels are elevated in T2DM patients and closely associated with glycolipid metabolism.Both indicators can serve as biomarkers for evaluating DR development in T2DM patients,with combined use providing enhanced predictive accuracy.关键词
