腹膜透析患者血清iPTH、TRF表达变化及其临床意义
Changes of Serum iPTH and Transferrin Expression in Peritoneal Dialysis Patients and Their Clinical Significance

Abstract
Objective To investigate the expression of intact parathyroid hormone(iPTH)and transferrin(TRF)in peritoneal dialysis(PD)patients and their relationship with inflammatory response and peritonitis.Methods A total of 120 PD patients admitted to the Renal Department of the Second People's Hospital of Fuyang City from January 2020 to June 2023 were selected as the study group.The patients were followed up for six months and categorized into the peritonitis group(n=23)and the non-peritonitis group(n=97)based on the occurrence of peritonitis.Simultaneously,120 age-and sex-matched healthy subjects were included as the control group.Serum levels of iPTH,TRF,and inflammatory factors[Hs-CRP,TNF-α,TNF-β,IL-6,IL-8]were compared between groups.Pearson correlation analysis was performed to evaluate the relationship between iPTH,TRF,and inflammatory factors.Multiple linear regression was used to identify factors influencing iPTH and TRF expression in PD patients.ROC curve analysis assessed the predictive value of serum iPTH and TRF for peritonitis in PD patients.Results Serum iPTH levels in the study group were significantly higher,while TRF levels were significantly lower compared to the control group(P<0.05).In PD patients,iPTH levels positively correlated with Hs-CRP,TNF-α,TNF-β,IL-6,and IL-8(P<0.05)but negatively correlated with IL-13(P<0.05).TRF levels negatively correlated with Hs-CRP,TNF-α,TNF-β,IL-6,and IL-8(P<0.05),but positively correlated with IL-13(P<0.05).Multiple linear regression analysis revealed that elevated levels of Hs-CRP,TNF-α,TNF-β,IL-6,IL-8,and reduced IL-13 were factors affecting iPTH and TRF expression(P<0.05).Serum iPTH levels were higher,and TRF levels were lower in the peritonitis group compared to the non-peritonitis group(P<0.05).ROC curve analysis showed that the AUCs for predicting peritonitis using serum iPTH and TRF were 0.928 and 0.869,respectively,while their combined AUC was 0.961.Conclusion Serum iPTH levels are elevated,and TRF levels are decreased in PD patients,both closely related to microinflammatory response and prognosis.These biomarkers can aid in evaluating peritonitis risk and patient outcomes.关键词
