The Predictive Value of Fetal Ductus Venosus Blood Flow and Uterine Artery Blood Flow Parameters Assessed by Ultrasound in Fetal Growth Restriction of Pregnant Women with Gestational Hypertension

Abstract
Objective To evaluate the impact of fetal ductus venosus blood flow and uterine artery blood flow parameters assessed by ultrasound on fetal growth restriction in pregnant women with gestational hypertension,thus providing reliable basis for early screening and clinical intervention.Methods A total of 89 fetuses delivered by pregnant women diagnosed with gestational hypertension in the Obstetrics Department of Chengde Central Hospital between October 2022 and September 2024 were retrospectively selected.Depending on the condition of fetal growth restriction,those with fetal growth restriction were included in the observation group(n=49),and the remaining were included in the control group(n=40).The peak diastolic to peak flow velocity ratio of fetal ductus venosus(DV-S/D),pulsatility index(DV-PI),resistance index(DV-RI),the ratio of venous catheter blood flow to umbilical vein blood flow(Qdv/Quv),the peak diastolic to flow velocity ratio of uterine artery blood flow(S/D),pulsatility index(PI),and resistance index(RI)were compared between groups.Multivariate logistic regression analysis was performed to identify influencing factors for fetal growth restriction in pregnant women with gestational hypertension.Receiver operating characteristic(ROC)curve was used to analyze the effectiveness of fetal ductus venosus blood flow and uterine artery blood flow parameters assessed by ultrasound in predicting fetal growth restriction of pregnant women with gestational hypertension.Results Uterine arterial flow parameters of pregnant women gestational hypertension(PI,RI,S/D)and fetal venous catheter blood flow parameters(DV-PI,DV-RI,DV-S/D,Qdv/Quv)were significantly higher in the observation group than those of the control group(P<0.05).Multivariate logistic regression analysis showed that uterine arterial flow parameters of pregnant women gestational hypertension(PI,RI,S/D)and fetal venous catheter blood flow parameters(DV-PI,DV-RI,DV-S/D,Qdv/Quv)were all risk factors for fetal growth restriction(P<0.05).ROC curve showed that the area under the curve(AUC)of fetal ductus venosus blood flow combined with uterine artery blood flow parameters assessed by ultrasound in predicting fetal growth restriction was 0.923,with the specificity of 0.857,and the sensitivity of 0.875.Conclusion Fetal ductus venosus blood flow and uterine artery blood flow parameters can be used as effective predictors of fetal growth restriction in pregnant women with gestational hypertension.Joint evaluation of these ultrasound hemodynamic parameters can improve the accuracy of early diagnosis of fetal growth restriction and provide strong support for the clinical management of patients with gestational hypertension.关键词
