Abstract
Objective To investigate the value of mean platelet volume(MPV)/lymphocyte(LYM)ratio(MPVLR)in predicting the prognosis of radiotherapy for locally advanced cervical cancer(LACC).Methods A total of 132 LACC patients admitted to the Department of Radiology,Yuncheng Central Hospital Affiliated to Shanxi Medical University from July 2019 to July 2022 were selected as the research objects.All cases received radiotherapy or concurrent chemoradiotherapy.The automatic blood cell analyzer was used to detect MPV and LYM of LACC patients before treatment and calculate MPVLR.According to the optimal cut-off value of MPVLR determined by the receiver operating characteristic(ROC)curve,132 LACC patients were divided into the high-MPVLR group and low-MPVLR group.The clinical characteristics of patients were compared between groups.Multivariate Cox regression was used to analyze the risk factors affecting the prognosis of radiotherapy for LACC.Results The ROC curve showed that the area under the curve(AUC)of MPVLR in predicting the prognosis of radiotherapy for LACC was 0.792(95%CI 0.742-0.837),with the optimal cut-off value of 5.13,the sensitivity of 92.31%,and the specificity of 64.29%.According to the optimal cut-off value of MPVLR,132 LACC cases were divided into the high-MPVLR group(MPVLR≥5.13,n=54)and low-MPVLR group(MPVLR<5.13,n=78).The proportion of tumor diameter>50 mm and the proportion of poorly differentiated cases in the high-MPVLR group were significantly higher than those in the low-MPVLR group(P<0.05).A total of 132 LACC cases were followed up for 2 years after radiotherapy,of which 109 cases survived and 23 cases died.The 2-year overall survival of LACC after radiotherapy was 82.58%(109/132).The 2-year overall survival of LACC cases with International Federation of Gynecology and Obstetrics(FIGO)stage Ⅱ b-Ⅲ A,tumor diameter≤50 mm,moderate/high differentiation,and MPVLR<5.13 was significantly higher than that of cases with FIGO stage Ⅲ B-Ⅳ A,tumor diameter>50 mm,poor differentiation,and MPVLR≥5.13(all P<0.05).Multivariate Cox regression analysis showed that FIGO stage ⅢB-ⅣA(HR=2.307,95%CI:1.346-3.955),poorly differentiated(HR=2.646,95%CI:1.602-4.370),and MPVLR≥5.13(HR=2.998,95%CI:1.798-5.001)were the risk factors for the prognosis of radiotherapy for LACC(P<0.05).Conclusion MPVLR can be used as a clinical index to evaluate the prognosis of radiotherapy for LACC,and the high MPVLR is a risk factor for the prognosis of radiotherapy for LACC.关键词
