Abstract
Objective To analyze the influencing factors for infections after laparoscopic surgery in patients with obstructive colorectal cancer(CRC).Methods A total of 103 patients with postoperative infections after laparoscopic surgery for obstructive CRC in the Department of General Surgery,Chui yang liu Hospital Affiliated to Tsinghua University from June 2021 to July 2024 were retrospectively recruited in the infection group.During the same period,90 obstructive CRC patients who were treated with laparoscopic surgery without developing postoperative infections were included in the control group.Location of infection and the distribution of pathogenic bacteria in the infection group were recorded.The admission records,hospitalization medical records and other clinical data of the two groups of patients were collected and sorted.Multivariate logistic regression analysis was performed to identify influencing factors for infections after laparoscopic surgery in patients with obstructive CRC.Results In the infection group,there were 48(46.60%)cases of incisional infections,45(43.69%)cases of abdominal infections,8(7.77%)cases of nosocomial pneumonia,and 2(1.94%)cases of urinary tract infections.A total of 103 pathogenic bacteria were detected,including 65(63.11%)Gram-negative bacteria,35(33.98%)Gram-positive bacteria,and 3(2.91%)fungi.The proportion of patients with diabetes,hypoalbuminemia,anemia,and operation time>3 hours in the infection group was significantly higher than that in the control group(χ2=36.739,6.686,6.413,and 40.350,respectively;P<0.05).Multivariate logistic regression analysis showed that diabetes,hypoalbuminemia,and operation time>3 hours were independent risk factors for infections after laparoscopic surgery in patients with obstructive CRC(P<0.05).Conclusion Infections after laparoscopic surgery in patients with obstructive CRC are mainly incisional infection and abdominal infection,and the pathogenic bacteria causing infection are mainly Gram-negative bacteria.Diabetes,hypoalbuminemia,and operation time>3 hours are independent risk factors for infection after laparoscopic surgery in patients with obstructive CRC.关键词
