Abstract
Objective To explore the value of mean platelet volume/lymphocyte ratio(MPVLR)in predicting deep vein thrombosis(DVT)in patients with traumatic lower limb fractures(TLLFs).Methods A total of 96 patients with TLLFs from May 2022 to December 2023 and received surgical treatment in the Mingshan District People's Hospital of Ya'an City(Mingshan Hospital,First Affiliated Hospital of Chengdu Medical College)were selected as the study objects.They were divided into thrombus group(n=33)and non-thrombus group(n=63)according to the occurrence of postoperative DVT.After admission,the mean platelet volume(MPV)and lymphocyte count(LYM)were measured,and MPVLR was calculated.General data,biochemical indicators and MPVLR were analyzed and compared.The predictive value of MPVLR in patients with TLLFs was evaluated by the receiver operating characteristic(ROC)curve.The related factors affecting DVT in patients with TLLFs were analyzed by multivariate Logistic regression.Results The operative time of the thrombus group was significantly longer than that of the non-thrombus group,and the Caprini thrombus risk score and peripheral blood white blood cell count(WBC),platelet count(PLT),fibrinogen(FIB),MPV,MPVLR,D-dimer(D-D),prothrombin time(PT)and activated partial thromboplastin time(APTT)were significantly higher compared with non-thrombus group,while the LYM was significantly lower compared with non-thrombus group(P<0.05).ROC curve Results showed that the area under the curve(AUC)of MPVLR in predicting DVT in patients with TLLFs was 0.886(95%CI:0.836-0.936).Multivariate regression analysis showed that Caprini thrombus risk score>6.52(OR=3.504,95%CI:1.943-6.322)and PLT>310.22×109/L(OR=1.984,95%CI:1.496-2.631),D-D>1.02mg/L(OR=2.591,95%CI:1.740-3.857),APTT>35.62 s(OR=2.428,95%CI:1.637-3.600),and MPVLR>4.36(OR=3.916,95%CI:2.083-7.360)were risk factors for DVT in patients with TLLFs(P<0.05).Conclusion MPVLR is abnormally elevated in patients with DVT and TLLFs,and can be used as a risk factor for the occurrence of DVT.Meanwhile,it has good predictive value for the occurrence of DVT.关键词
