Abstract
Objective To analyze the relationship between peripheral blood T lymphocyte subsets and response to argon-helium knife cryoablation for hepatocarcinoma.Methods A total of 90 patients with liver cancer admitted to Lu'an People's Hospital from January 2020 to January 2024 were selected.According to the degree of tumor necrosis,the patients were divided into complete ablation group and incomplete ablation group,and the clinical data of the two groups were compared.Multivariate Logistic regression was used to analyze the related factors affecting the postoperative short-term efficacy of argon-helium knife cryoablation for hepatocarcinoma.ROC curve analysis of peripheral blood T lymphocyte subsets was used to predict the postoperative short-term efficacy of argon-helium knife cryoablation for hepatocarcinoma.Kaplan-Meier method was used to draw the survival curve,and Log-rank was used to test and compare the difference of the survival curve.Results There was no significant difference in gender,age,history of hypertension,history of diabetes,cirrhosis and number of tumors between the complete ablation group and the incomplete ablation group(P>0.05).There were significant differences in tumor diameter,CD3+,CD4+,CD8+and CD4+/CD8+between the two groups(P<0.05).Binary Logistic regression analysis showed that larger tumor diameter,lower CD3+,lower CD4+,higher CD8+and lower CD4+/CD8+were independent risk factors for the postoperative short-term efficacy of argon-helium knife cryoablation for hepatocarcinoma(P<0.05).ROC curve results showed that the area under the curve of CD3+,CD4+,CD8+and CD4+/CD8+were 0.803,0.818,0.792 and 0.804,respectively(P<0.05),and the area under the curve of combined detection was higher than that of single detection.The results indicated that CD3+,CD4+,CD8+,CD4+/CD8+combined detection could be used as an effective index to predict the postoperative short-term efficacy of argon-helium knife cryoablation for hepatocarcinoma.After 1 year of follow-up,14 patients(15.56%)died.There was significant difference in survival curves between the complete ablation group and the incomplete ablation group(P<0.05).Conclusions Peripheral blood T lymphocyte subsets can be used as potential indexes to evaluate response to argon-helium knife cryoablation for hepatocarcinoma and prognosis of patients.In addition,larger tumor diameter,lower CD3+,lower CD4+,higher CD8+,and lower CD4+/CD8+were all independent risk factors that affected the postoperative short-term efficacy of patients.关键词
