Abstract
Objective To explore the correlation of systemic immune-inflammation index(SII)with the condition and prognosis of psoriasis vulgaris(PV).Methods Clinical data of 193 patients with PV admitted to the Zigong Fourth People's Hospital from January 2022 to October 2024 were retrospectively analyzed.According to the Psoriasis Area and Severity Index(PASI),the enrolled PV patients were divided into the mild group(n=97),moderate group(n=75),and severe group(n=21).According to the degree of regression of skin lesions after 3 months of treatment,they were divided into the good prognosis group(n=106)and poor prognosis group(n=87).Neutrophil count(NEU),lymphocyte count(LYM),and platelet count(PLT)on admission were examined,and SII was calculated.Pearson correlation analysis was used to examine the correlation between SII and PASI.Receiver operator characteristic(ROC)curve analysis was used to evaluate the prognostic value of SII in PV.Multivariate logistic stepwise regression analysis was conducted to identify the influencing factors for the prognosis of PV.Results PLT,NEU and SII were significantly higher in the severe group,followed by the moderate group and mild group,while the LYM was significantly higher in the mild group,followed by the moderate group and severe group(P<0.05).Correlation analysis showed that SII was positively correlated with PASI in PV patients(P<0.05).PLT,NEU,and SII in the poor prognosis group were significantly higher than those of the good prognosis group,and LYM was significantly lower(all P<0.05).The area under the curve(AUC,95%CI)of PLT,NEU,LYM,and SII in predicting the prognosis of PV was 0.728(0.658-0.782),0.835(0.769-0.889),0.792(0.741-0.845),and 0.909(0.852-0.971),respectively.The proportions of people with smoking history,drinking history,hypoproteinemia and fatty liver in the poor prognosis group were significantly higher than those of the good prognosis group(all P<0.05).Hypoproteinemia(OR=2.582,95%CI:1.286-5.162),fatty liver(OR=2.817,95%CI:1.229-6.596),SII≥365.83(OR=4.324,95%CI:2.124-8.615)were factors influencing the prognosis of PV(all P<0.05).Conclusion SII increases in PV patients,and is related to the severity and prognosis of the disease.It is expected to be a biological indicator for evaluating the prognosis of PV.关键词
